摘要
目的评估血清胃泌素17(G-17)、胃蛋白酶原(PG)对早期胃癌(EGC)筛查的价值。方法收集西安医学院第一附属医院消化内科于2018年9月~2020年5月因消化道症状收住入院经电子胃镜及病理检查确诊的早期胃癌患者22例、胃癌癌前病变患者113例,包括胃息肉患者40例、胃溃疡患者37例、慢性萎缩性胃炎患者36例。分别设为早期胃癌组、癌前病变组包括胃息肉(GP)组、胃溃疡(GU)组、慢性萎缩性胃炎(CAG)组,另选取50例胃粘膜正常或浅表性胃炎者设为对照组。比较各组血清G-17、PG、PGR(PGⅠ/PGⅡ)表达水平,不同指标单独检测及联合检测诊断早期胃癌的价值。结果早期胃癌组和胃溃疡组PG水平高于对照组、胃息肉组及慢性萎缩性胃炎组;早期胃癌组、胃溃疡组G-17水平高于对照组和胃息肉组;早期胃癌组、慢性萎缩性胃炎组PGR低于对照组和胃息肉组,差异有统计学意义(P<0.05)。PG联合G-17诊断EGC的ROC曲线(AUC)为0.813,灵敏性(SEN)为81.80%,特异性(SPE)为78.50%。结论联合检测PG、G-17在EGC的筛查中具有较高的应用价值,有助于提高早期胃癌的检出率。
Objective To evaluate the value of serum gastrin 17(G-17)and pepsinogen(PG)in screening early gastric cancer(EGC).Methods A collection of 22 patients with early gastric cancer and 113 patients with precancerous lesions of gastric cancer who were admitted to the hospital due to gastrointestinal symptoms from September 2018 to May 2020 in the First Affiliated Hospital of Xi’an Medical University and diagnosed by electronic gastroscopy and pathological examination.There were 40 patients with gastric polyps,37 patients with gastric ulcer,and 36 patients with chronic atrophic gastritis.Set as early gastric cancer group and precancerous lesion group including gastric polyp(GP)group,gastric ulcer(GU)group,chronic atrophic gastritis(CAG)group,and 50 cases with normal gastric mucosa or superficial gastritis were selected as control group.The serum G-17,PG,and PGR(PGⅠ/PGⅡ)expression levels in each group were compared,and the value of separate and combined detection of different indicators in the diagnosis of early gastric cancer.Results PG levels in the early gastric cancer group and gastric ulcer group were higher than those in the control group,gastric polyp group and chronic atrophic gastritis group;G-17 levels in the early gastric cancer group and gastric ulcer group were higher than those in the control group and gastric polyp group;The PGR of the early gastric cancer group and chronic atrophic gastritis group was lower than that of the control group and gastric polyp group,the difference was statistically significant(P<0.05).The ROC curve(AUC)of PG combined with G-17 to diagnose EGC was 0.813,the sensitivity(SEN)was 81.80%,and the specificity(SPE)was 78.50%.Conclusion The combined detection of PG and G-17 has a high application value in the screening of EGC,which helps to improve the detection rate of early gastric cancer.
