特利加压素联合奥美拉唑治疗肝硬化伴食管-胃底静脉曲张破裂出血疗效分析

Efficacy of Terlipressin Combined with Omeprazole in Treatment of Liver Cirrhosis with Esophagogastric Variceal Bleeding

目的:分析特利加压素联合奥美拉唑治疗肝硬化伴食管-胃底静脉曲张破裂出血的疗效。方法:选取本院2015年11月—2019年11月期间收治的肝硬化伴食管-胃底静脉曲张破裂出血患者80例,采用随机数字表法分为两组,各40例。对照组采用奥曲肽联合奥美拉唑治疗,观察组采用特利加压素联合奥美拉唑治疗,比较两组止血效果、血流动力学指标及胃肠激素水平。结果:观察组止血成功率高于对照组,差异有统计学意义(P<0.05);治疗后,两组中心静脉压(CVP)、门静脉血流速率(VPV)比较,差异无统计学意义(P>0.05);观察组治疗后门静脉压(PVP)、门静脉内径(DPV)低于对照组,差异有统计学意义(P<0.05);治疗后,观察组胰高血糖素(GLC)、胃泌素(GAS)均低于对照组,差异有统计学意义(P<0.05)。结论:特利加压素联合奥美拉唑治疗可提高肝硬化伴食管-胃底静脉曲张破裂出血患者止血成功率,改善血流动力学指标,调节胃肠激素水平。

Objective:To analyze the efficacy of terlipressin combined with omeprazole in treatment of liver cirrhosis with esophagogastric variceal bleeding.Methods:80 patients with liver cirrhosis with esophagogastric variceal bleeding who admitted in the hospital from November 2015 to November 2019 were divided into two groups according to random number table,with 40 cases in each group.Control group was treated with octreotideand omeprazole,and observation group was treated with terlipressin and omeprazole.The hemostatic effect,hemodynamic indexes and gastrointestinal hormone levels were compared between the two groups.Results:The success rate of hemostasis in observation group was higher than that in control group(P<0.05);After treatment,there was no statistical difference in the central venous pressure(CVP)and velocity of the main portal vein between two groups(P>0.05);After treatment,the portal vein pressure(PVP)and diameter of portal vein(DPV)in observation group were lower than those in control group(P<0.05);After treatment,the glucagon(GLU)and gastrin(GAS)in observation group were lower than those in control group(P<0.05).Conclusion:Terlipressin combined with omeprazole in treatment of liver cirrhosis with esophagogastric variceal bleeding can increase the success rate of hemostasis,improve hemodynamic indexes and regulate gastrointestinal hormone levels.
