microRNA-21在胃癌患者血清中的表达及临床意义

Expression and clinical significance of serum microRNA-21 in gastric cancer

目的分析microRNA-21在胃癌患者血清的表达及临床意义。方法前瞻性选取首都医科大学附属北京友谊医院消化分中心60例胃癌患者及健康体检中心年龄性别匹配的健康对照人群60例的临床资料。提取胃癌患者和健康对照人群的血清RNA,用荧光定量聚合酶链式反应(RT-PCR)的方法分析microRNA-21的表达。血清microRNA-21的与胃癌患者临床病理特征的关系;并采用受试者工作特征(ROC)曲线计算microRNA-21对胃癌的诊断价值。结果胃癌患者血清microRNA-21相对表达量明显高于正常人群(3.66±1.77 vs.2.17±1.28,P<0.01),microRNA-21的表达与胃癌患者临床分期、淋巴结转移及远处转移明显相关(P<0.05),但与患者性别、年龄、肿瘤分化程度无关(P>0.05)。以2.56为临界值,血清microRNA-21相对表达量诊断胃癌的敏感度为71.7%,特异度为68.3%。结论胃癌患者血清microRNA-21水平上调,在辅助胃癌诊断中具有一定的指导价值。

Objective To analyze the expression of serum microRNA-21 in patients with gastric cancer and its clinical significance.Methods Sixty patients with gastric cancer and 60 age and sex-matched healthy individuals from department of gastroenterology and health check center of Beijing Friendship Hospital,Capital Medical University were prospectively enrolled.Serum RNA was extracted,and then stem-loop real-time reverse transcription-polymerase chain reaction(RT-qPCR)was used to detect the expression level of microRNA-21.To detect the expression of serum microRNA-21 in patients with gastric cancer and in healthy individuals,and to statistically analyze the association of the expression with clinicopathological features.The diagnostic value of microRNA-21 for gastric cancer was calculated using the receiver operating characteristic(ROC)curve.Results The expression of serum microRNA-21 was statistically higher in patients with gastric cancer than in healthy individuals(3.66±1.77 vs.2.17±1.28,P<0.01).For patients with gastric cancer,the relative expression of microRNA-21 was closely associated with tumor stage,lymph node metastasis,and distant metastasis(P<0.05),but no statistical difference was found between the relative expression of microRNA-21 and age,sex or differentiation(P>0.05).With the cut-off value of 2.56,the sensitivity and specificity of serum microRNA-21 relative expression for the diagnosis of gastric cancer was 71.7%and 68.3%,respectively.Conclusion The expression of serum microRNA-21 was higher in gastric cancer patients than in healthy individuals.The detection of microRNA-21 in serum might serve as a new biological biomarker in the diagnosis of gastric cancer.
