56510例患者不规则抗体及特异性检测结果分析

Analysis of irregular antibodies and specific test results in 56510 patients

目的探讨不规则抗体筛查在临床输血中的重要性,指导临床安全输血。方法应用卡式微柱凝胶法对西安高新医院2010年1月至2019年12月56510例患者(包括门急诊患者、有备血需求住院患者及常规检测抗体筛查患者)进行不规则抗体检测。结果56510例不规则抗体筛查患者中检出170例,检出率为0.3%;经陕西省血液中心进行抗体鉴定,同种特异性抗体145例(85.3%),自身抗体25例(14.7%),同种特异性抗体分别为抗-E 62例(36.5%)、抗-C 5例(2.94%)、抗-D 13例(7.64%)、抗-cE 11例(6.47%)、抗-CE 3例(1.76%)、抗-Ce 10例(5.88%)、抗-M 17例(10.0%)、抗-N 3例(1.76%)、抗-Lea 10例(5.88%)、抗-Leb 3例(1.76%)、IgG-cE联合IgM-M 4例(2.35%)、IgG-CE联合IgM-E 4例(2.35%)。结论临床输血前不规则抗体检测对于减少溶血性输血反应、保障临床安全输血有重要意义。

Objective To explore the importance of irregular antibody screening in clinical blood transfusion,and to guide clinically safe blood transfusion.Methods Irregular antibody testing was carried out on 56510 patients(including outpatients and emergency patients,hospitalized patients with blood needs,and routine antibody screening patients)who admitted to Xi'an High-tech Hospital from January 2010 to December 2019 using the card micropillar gel method.Results A total of 170 cases of irregular antibody screening were detected in 56510 cases,with a detection rate of 0.3%;according to antibody identification of the Shaanxi Blood Center,there were 145 cases of specific antibodies(85.3%),25 cases of autoantibodies(14.7%),and the specific antibodies were Anti-E 62 cases(36.5%),anti-C 5 cases(2.94%),anti-D 13 cases(7.64%),anti-cE 11 cases(6.47%),anti-CE 3 cases(1.76%),anti-Ce 10 cases(5.88%),anti-M 17 cases(10.0%),Anti-N 3 cases(1.76%),anti-Lea 10 cases(5.88%),anti-Leb 3 cases(1.76%),IgG-cE combined with IgM-M 4 cases(2.35%),IgG-CE combined with IgM-E 4 cases(2.35%).Conclusion Irregular antibody detection before clinical blood transfusion is of great significance,which can reduce hemolytic transfusion reactions and ensure clinical safety of blood transfusion.
