米非司酮和米索前列醇在流产中的应用

目的在实施药物流产的过程中,对单纯应用米非司酮片(简称Ru486)与Ru486联合米索前列醇(以下简称MIS)流产成功率及安全性进行比较,研究联合用药的效果。方法将2018年4月至2020年3月来我院进行药物流产的1200例患者作为研究对象,采用计算机随机抽签的方式分成有600例患者的单一组和600例患者的联合组,单一组给予Ru486进行药物流产,联合组在单一组的基础上加用MIS,对比不同组之间的流产成功率、相关指标以及不良反应和并发症情况。结果单一组流产成功率(87.33%)低于联合组(96.33%),差异有统计学意义(P<0.05)。单一组出血量多于联合组,出血时间和胚囊排出时间长于联合组,有统计学意义(P<0.05)。单一组不良反应和并发症总发生率分别为13.83%和12.67%,均高于联合组(6.17%和6.33%),差异显著,有统计学意义(P<0.05)。结论随着流产人数的不断增多,药物流产因非侵入操作更受人们青睐,所以其应用越来越多且越普遍。联合用药有较为确切的临床效果,流产成功率高,不良反应少,不仅用时少而且可以有效降低出血量等,可以代替单一给药作为首选的药物流产方案。

Objective In the process of implementing medical abortion,to compare the abortion success rate and safety of simply using mifepristone tablets(abbreviated as Ru486)and Ru486+misoprostol(hereinafter referred to as MIS).The effect of combination medication.Methods One thousand and two hundred patients who came to our hospital for drug abortion from April 2018 to March 2020 were taken as the research objects,and they were divided into a single group of 600 patients and a combined group of 600 patients by random drawing by computer.The single group was given Ru486 for drug abortion,the combined group was based on a single group+MIS,and the abortion success rate,related indicators,and adverse reactions and complications between two groups were compared.Results The success rate of abortion in the single group was 87.33%lower than that of the combined group(96.33%),the difference was statistically significant(P<0.05).The amount of bleeding in a single group was more than that in the combined group,and the bleeding time and embryo sac discharge time were longer than those in the combined group(P<0.05).The total incidence of adverse reactions and complications in a single group were 13.83%and 12.67%,respectively,which were higher than those in the combined group(6.17%and 6.33%),and the difference was significant(P<0.05),which was statistically significant.Conclusion As the number of abortions continues to increase,drug abortion was more popular due to noninvasive operations,and drug abortion applications were more and more common.The single-dose method can no longer meet people's needs,and the combination of drugs has more precise clinical effects.The miscarriage rate was high and the adverse reactions are low.It not only takes less time but also effectively reduces the amount of bleeding.
