丙泊酚、芬太尼复合小剂量咪唑安定静脉麻醉在妇产科门诊手术中的应用

Application of Propofol, Fentanyl Combined with Low Dose Midazolam Intravenous Anesthesia in Outpatient Surgery ofObstetrics and Gynecology

目的探究妇产科门诊手术中采用丙泊酚、芬太尼复合小剂量咪唑安定静脉麻醉的效果。方法于2017年4月至2018年5月选取本院收治的72例妇产科门诊手术患者,并按照数字表法分为两组,对比组予以丙泊酚、芬太尼麻醉,试验组实施丙泊酚、芬太尼复合小剂量咪唑安定麻醉。并对两组患者麻醉不同时段平均动脉压、心率、呼吸、血氧饱和度水平、起效、手术与苏醒的时间与丙泊酚用量及不良反应发生率进行比较。结果两组患者麻醉后平均动脉压、心率、呼吸与血氧饱和度最低值比麻醉前低(P<0.05);对比组患者手术完成后呼吸与血氧饱和度值比试验组低(P<0.05);两组患者起效、手术以及苏醒的时间比较差异不显著(P>0.05);试验组患者丙泊酚用量比对比组少(P<0.05);对比组患者不良反应发生率25.00%,与试验组(19.44%)相比没有显著差异(P>0.05)。结论妇产科门诊手术中采用丙泊酚、芬太尼复合小剂量咪唑安定静脉麻醉效果较好,可减少丙泊酚用量,安全性较好。

Objective To explore the effect of intravenous anesthesia with propofol and fentanyl combined with small dose midazolam in gynecology and obstetrics outpatient surgery.Methods From April 2017 to May 2018,72 outpatients of obstetrics and gynecology admitted to our hospital were selected and divided into two groups according to the digital table method.The control group was anesthetized with propofol and fentanyl,while the experimental group was anesthetized with propofol and fentanyl combined with small dose midazolam.The average arterial pressure,heart rate,respiration,blood oxygen saturation level,onset of action,operation and recovery time,propofol dosage and incidence of adverse reactions were compared between the two groups.Results After anesthesia,the mean arterial pressure,heart rate,respiration and oxygen saturation of the two groups were lower than those before anesthesia(P<0.05).The respiratory and blood oxygen saturation values in the control group were lower than those in the experimental group(P<0.05).There was no significant difference in onset,operation and recovery time between the two groups(P>0.05).The dosage of propofol in the experimental group was less than that in the control group(P<0.05).The incidence of adverse reactions in the control group was 25.00%,which was no significant difference compared with experiment group(19.44%)(P>0.05).Conclusion Intravenous anesthesia with propofol and fentanyl combined with small dose of midazolam is effective,can reduce the dosage of propofol,and was safe.
