肺炎克雷伯菌医院感染及耐药性分析

Analysis of nosocomial infection and drug resistance of klebsiella pneumonia

目的:分析肺炎克雷伯菌医院感染分布情况及耐药性,为临床选用抗菌药提供参考。方法:选取我院2017年4月〜2019年10月送检的脓液、血液、痰液、尿液等感染性标本,对所有标本进行细菌培养及耐药性测试,分析肺炎克雷伯菌于医院各科室分布情况及耐药性。结果:2017年4月〜2019年10月送检标本中共检出2157株肺炎克雷伯菌,总检出率为11.48%;肺炎克雷伯菌于ICU科室中检出率占比最高,为20.26%,其次为呼吸内科,占15.11%,综合内科,占12.15%;肺炎克雷伯菌多分布于痰液及尿液中,其检出率分别为76.59%、8.95%;肺炎克雷伯菌对头孢菌素、碳青霉烯类抗生素等耐药率较低,对其他药物耐药性较高。结论:肺炎克雷伯菌于医院感染中多分布于ICU、呼吸内科及综合内科,多存在于痰液、尿液中,而其对头孢菌素、碳青霉烯类抗菌药耐药率较低,临床需做好预防措施,并合理选用抗生素,有效控制感染情况。

Objective:To analyze the distribution and drug resistance of klebsiella pneumoniae nosocomial infection,so as to provide reference for clinical selection of antimicrobial drugs.Methods:Samples of pus,blood,sputum,urine and other infectious diseases sent for examination in our hospital from April 2017 to October 2019 were selected.All samples were cultured and tested for bacterial resistance,and the distribution and drug resistance of klebsiella pneumoniae in all departments of the hospital were analyzed.Results:From April 2017 to October 2019,2157 strains of klebsiella pneumoniae were detected,with a total detection rate of 11.48%.Klebsiella pneumoniae had the highest detection rate in ICU departments(20.26%),followed by respiratory department(15.11%)and general department(12.15%).Klebsiella pneumoniae was mainly distributed in sputum and urine,and the detection rates were 76.59%and 8.95%,respectively.The drug resistance rate of klebsiella pneumoniae to cephalosporins,carbapenems and other antibiotics was lower,and the drug resistance to other drugs was higher.Conclusion:Klebsiella pneumoniae in throughout in the ICU hospital infection,respiratory medicine and comprehensive internal medicine,found in sputum,urine,and the prevalence of ampicillin resistance was 100.00%,the adversary toxin,carbon green alkene antimicrobial drug resistant rate is low.The preventive measures and reasonable use of antibiotics should be used in clinical to get the infection effectively controlled.
