塞来昔布联合化疗治疗晚期肺鳞癌的临床观察

Clinical Observation on Celecoxib Combined with Chemotherapy in the Treatment of Patients with Advanced Lung Squamous Cell Carcinoma

目的观察环氧化酶-2(COX-2)抑制剂塞来昔布联合GP化疗方案(吉西他滨+顺铂)治疗晚期肺鳞癌的临床疗效和安全性。方法将50例晚期肺鳞癌患者随机分为对照组和观察组各25例,对照组仅用吉西他滨和顺铂治疗,观察组采用塞来昔布联合吉西他滨和顺铂治疗,比较两组的疗效及不良反应。结果第4~6个化疗周期结束后,两组的ORR(44.0%vs.36.0%)比较无统计学差异(P>0.05)。观察组的DCR为92.0%,高于对照组的36.0%(P<0.05)。观察组的1年生存率为64.0%,高于对照组的20.0%(P<0.05)。两组的骨髓抑制、胃肠道反应和乏力发生率比较无统计学差异(P>0.05)。结论塞来昔布联合GP化疗方案治疗晚期肺鳞癌患者的安全性高,可提高患者的临床疗效。

Objective To observe the clinical efficacy and safety of cyclooxygenase-2(COX-2)inhibitor celecoxib combined with GP chemotherapy(gemcitabine plus cisplatin)in the treatment of advanced lung squamous cell carcinoma.Methods 50 patients with advanced lung squamous cell carcinoma were randomly divided into control group and observation group,with 25 cases in each group.The control group was only treated with gemcitabine and cisplatin,while the observation group was treated with celecoxib combined with gemcitabine and cisplatin.The efficacy and adverse reactions of the two groups were compared.Results After the 4^th to 6^th chemotherapy cycle,no statistical difference was found in the ORR(44.0%vs.36.0%)between the two groups(P>0.05).The DCR of the observation group was 92.0%,higher than 36.0%of the control group(P<0.05).The 1-year survival rate of the observation group was 64.0%,higher than 20.0%of the control group(P<0.05).No statistical difference was found in the incidences of myelosuppression,gastrointestinal reactions and fatigue between the two groups(P>0.05).Conclusions Celecoxib combined with GP chemotherapy has high safety in the treatment of patients with advanced lung squamous cell carcinoma,and can improve the clinical efficacy.
