Rowell综合征1例

A Case of Rowell′s Syndrome

患者女,22岁,全身红斑、丘疹伴痒痛8 d,发热2 d。抗核抗体示ANA 1∶160斑点型,抗U1-RNP(3+),抗Sm(3+),抗SSA(3+),抗Ro-52(+)。右侧大腿皮损组织病理示:表皮角化不全,棘层轻度增厚,棘层上部见少许中性粒细胞,灶性基底细胞空泡变性,真皮乳头及浅层血管扩张、充血,周围少许淋巴细胞、组织细胞及中性粒细胞、核尘浸润,少许红细胞外溢。免疫荧光基底膜带IgG线状沉积。根据临床表现、组织病理、免疫荧光诊断:Rowell综合征。

A 22-year-old female presented systemic erythema and papules with itching and pain for 8 days,also she had a fever for 2 days.Antinuclear antibody showed ANA 1∶160 nuclear spotted type,anti-U1-RNP(3+),anti-Sm(3+),anti-SSA(3+),anti-Ro-52(+).A biopsy pecimen obtained from her right thigh revealed epidermal keratinosis,slight thickening of the spinous layer,a few of neutrophils observed at the upper part of the spinous layer,vacuolation of focal basal cells,dilation and congestion of the dermal papilla and superficial blood vessels,infiltration of a few lymphocytes,tissue cells,neutrophils,nuclear dust and a few red blood cells.Immunofluorescence showed linear deposition of IgG in basement membrane.Based on clinical,histopathological and immunofluorescence findings,the diagnosis of Rowell′s syndrome was made.
