Lp-PLA2与脑梗死患者颈动脉斑块稳定性的关系研究

Relationship between Lp-PLA2 and stability of carotid plaque in patients with cerebral infarction

目的探讨血清脂蛋白相关磷脂酶A2(Lp-PLA2)与脑梗死患者颈动脉斑块稳定性的关系。方法选取该院2019年3月至2020年3月收治的110例脑梗死、100例非脑梗死住院患者分别纳入脑梗死组、非脑梗死组,均予以Lp-PLA2水平检测及颈动脉超声检查。结果脑梗死组患者Lp-PLA2水平高于非脑梗死组,差异有统计学意义(P<0.05);两组有颈动脉斑块患者Lp-PLA2水平均高于无斑块患者,差异均有统计学意义(P<0.05);脑梗死颈动脉斑块不稳定者Lp-PLA2水平高于脑梗死斑块稳定者,差异有统计学意义(P<0.05);神经功能损伤程度评分(NHSS评分)>20分者Lp-PLA2水平均高于NHSS评分为2~5、6~15、16~20分者,差异均有统计学意义(P<0.05);脑梗死组复发、病情进展者Lp-PLA2水平高于初发、无进展者,差异均有统计学意义(P<0.05)。结论Lp-PLA2水平升高与脑梗死斑块生成及其稳定性存在关系;Lp-PLA2水平与脑梗死患者神经功能损伤程度呈正相关;Lp-PLA2水平升高,脑梗死患者复发、病情进展风险增加。

Objective To investigate the relationship between serum lipoprotein-associated phospholipase A2(Lp-PLA2)and carotid plaque stability in patients with cerebral infarction.Methods A total of 110 cerebral infarction patients and 100 non-cerebral infarction inpatients in the hospital from March 2019 to March 2020 were enrolled into the cerebral infarction group and non-cerebral infarction group respectively,and all of them were examined by Lp-PLA2 detection and carotid ultrasound.Results The level of Lp-PLA2 in cerebral infarction group was higher than that in non-cerebral infarction group,and the difference was statistically significant(P<0.05).The levels of Lp-PLA2 in patients with carotid plaque in both groups were higher than those without plaque,and the difference was statistically significant(P<0.05).The level of Lp-PLA2 in the unstable carotid plaque group was higher than that in the stable plaque group,and the difference was statistically significant(P<0.05).Lp-PLA2 levels of those with nerve function injury score(NHSS score)>20 were all higher than those with NHSS score 2-5,6-15,16-20,and the difference was statistically significant(P<0.05).The level of Lp-PLA2 in the recurrent and progressive cerebral infarction group was higher than that in the primary and non-progressive cerebral infarction group,and the difference was statistically significant(P<0.05).Conclusion There is a relationship between the increase of Lp-PLA2 level and the formation and stability of cerebral infarction plaque.Lp-PLA2 level is positively correlated with the degree of nerve function injury in cerebral infarction patients.Increased Lp-PLA2 level increases the risk of recurrence and disease progression in cerebral infarction patients.
