四维盆底超声检测初产妇盆底功能障碍性疾病的使用价值

Value of Detect Pelvic Floor Dysfunction in Primipara by Fourdimensional Pelvic Floor Ultrasound

目的探讨四维盆底超声在初产妇盆底功能障碍性疾病中的使用价值。方法选取2018年3月—2019年5月间本院收治的初产妇盆底功能障碍性患者共82例,根据其不同分娩方式分为A组(剖宫产组,50例)及B组(阴道分娩组,32例)。两组患者均行四维盆底超声进行检查,对比观察两组患者Green分型、Valsalva期BSD、尿道膀胱后角、膀胱颈移动度、尿道旋转角等指标。结果A组患者GreenⅠ型率高于B组、Ⅱ、Ⅲ型率低于B组,差异具有统计学意义(P<0.05);A组患者静止状态膀胱颈移动度及尿道膀胱后角与B组无明显差异(P>0.05);A组患者Valsalva期膀胱颈移动度、尿道膀胱后角及尿道旋转角均明显低于B组,差异具有统计学意义(P<0.05)。结论使用四维盆腔超声对初产妇盆底功能障碍性疾病患者进行检测效果理想,可帮助了解产妇分娩后盆底肌群的功能情况,为临床诊疗提供依据。

Objective To explore the value of 4 D pelvic floor ultrasound in pelvic floor dysfunction of primipara.Methods 82 patients with pelvic floor dysfunction from March 2018 to May 2019 were selected and divided into group A(cesarean section group,50 cases)and group B(vaginal delivery group,32 cases).All patients were examined by four-dimensional pelvic floor ultrasound,compared and observed the Gren classification,Valsalva stage BSD,posterior urethral bladder angle,bladder neck mobility,urethral rotation angle and other indexes.Results The rate of GreenⅠtype in group A was higher than that of group B,and the rate of typeⅡandⅢwas lower than that of group B.The difference was statistically significant(P<0.05);the static bladder neck mobility and posterior angle of urethra and bladder in group A were compared with group B.There was no significant difference(P>0.05);the movement of the bladder neck,the posterior angle of the urethra and bladder,the rotation angle of the urethra in group A were significantly lower than those in group B in Valsalva stage,and the difference was statistically significant(P<0.05).Conclusion The use of four-dimensional pelvic ultrasound to detect patients with pelvic floor dysfunction in primipara has an ideal effect,which can help understand the function of the pelvic floor muscles after delivery and provide a basis for clinical diagnosis and treatment.
