Predictive effects of family status and traumatic experiences on behavioral problems in middle school students

摘要 目的探讨家庭状况和儿童创伤经历对中学生行为问题的预测作用,为制定预防干预措施提供依据。方法2019年3月至5月采用随机抽样法抽取广州市某中学的1337名中学生,采用儿童期创伤问卷(CTQ)对中学生进行问卷调查;同时采用自制的家庭状况问卷和父母用儿童行为量表(CBCL)对学生父母进行问卷调查,应用逐步Logistic回归分析筛查确定中学生行为问题的预测因子后,进行重复抽样内部验证评估总预测效应。结果在1337名中学生中,行为问题总检出率为13.84%(185人),其中内化行为问题检出率为11.52%(154人),外化行为问题检出率为5.39%(72人)。行为问题预测因子为:中学生性别(女孩)(β=0.61)、学习差(β=1.40)、经常生病(β=0.95)、患有慢性病(β=0.74)、有创伤经历(β=0.38)、父母均饮酒(β=0.82)、亲子关系差(β=1.29)、父母关系差(β=0.87)、家庭月收入超过10000元(β=0.50)、家长曾心灵受伤(β=0.77),均P<0.05;内部验证显示受试者工作特征曲线下面积(AUC)为0.76(95%CI:0.72~0.79),P<0.01。结论家庭状况和儿童创伤经历对广州市中学生行为问题具有良好的预测效应。 Objective To investigate the predictive effect of family status and childhood traumatic experiences on behavioral problems of middle school students,so as to provide basis for the development of preventive interventions.Methods From March to May 2019,1337 students from middle school in Guangzhou were selected by random sampling method.All of them were investigated using Childhood Trauma Questionnaire(CTQ),and their parents were also investigated by self-designed family status questionnaire and Achenbach Child Behavior Checklist(CBCL).Stepwise Logistic regression was performed to select predictors of behavioral problems in middle school students,and bootstrap sampling internal validation was used to evaluate the predictive effect.Results Among the 1337 middle school students,the overall detection rate of behavioral problems was 13.84%(185),among which the detection rate of internalizing behavioral problems was 11.52%(154)and that of externalizing behavioral problems was 5.39%(72).Predictors of behavioral problems included gender of middle school students(females)(β=0.61),poor learning(β=1.40),frequent illness(β=0.95),chronic illness(β=0.74),with traumatic experience(β=0.38),both parents drink alcohol(β=0.82),poor parent-child relationship(β=1.29),poor mother-father relationship(β=0.87),family monthly income exceeding 10,000 yuan(β=0.50),traumatized parents(β=0.77)(all P<0.05).Internal verification showed that the area under curve(AUC)was 0.76(95%CI:0.72-0.79).Conclusion Family status and childhood traumatic experiences have good predictive effects on behavioral problems of middle school students in Guangzhou.
