改良Y型纳米骨板在颈后路单开门椎管扩大成形术中的应用研究

摘要 目的:探讨改良Y型纳米骨板(A组)和普通Y型纳米骨板(B组)在颈后路单开门椎管扩大成形术中的应用和临床疗效。方法:回顾性分析于2014年1月至2018年1月在我院行颈后路椎板扩大成形术患者,纳入符合标准,A组57例、B组41例。收集入组患者的一般资料、影像学资料、手术情况、术后随访记录,应用SPSS软件对相关数据作统计学分析。结果:A组手术时间明显少于B组(P<0.05);A组术后C5椎管矢状径明显大于B组(P<0.05);A组假体匹配程度良好率大于B组(P<0.05);手术出血量、颈椎生理曲度、术后感染发生率、术后轴性症状、术后C5神经根麻痹、同期神经恢复情况显示两组差异无统计学意义(P>0.05)。结论:A组与B组在多节段脊髓型颈椎病的治疗中具有疗效确切,但前者可缩短手术时间、增加C5椎管扩大直径以及增加假体与开门椎板间匹配程度。 Objective:To investigate the application of modified Y-shaped nano bone plate(group A)and common Ytype nano bone plate(group B)in posterior cervical open-door laminoplasty and clinical efficacy.Methods:Retrospectively analyzed the patients who underwent posterior cervical laminoplasty in our hospital from January 2014 to January 2018,including 57 cases in Group A and 41 cases in Group B.The general data,imaging data,surgical conditions and postoperative follow-up records of the patients were collected and analyzed by SPSS software.Results:The operation time of Group A was significantly shorter than that of Group B(P<0.05);the expansion of sagittal diameter of C5 spinal canal in Group A was significantly greater than that in Group B(P<0.05);the good rate of prosthesis matching in Group A was higher than that in Group B(P<0.05);there was no significant difference in the amount of bleeding,physiological curvature of cervical spine,incidence of postoperative infection,axial symptoms,postoperative C5 nerve root paralysis and nerve recovery in the same period between the two groups(P>0.05).Conclusion:Group A and Group B have definite curative effect in the treatment of multilevel cervical spondylotic myelopathy,but the former can shorten the operation time,increase the diameter of C5 spinal canal and increase the matching degree between prosthesis and open-door lamina.
