家庭医生签约服务在老年高血压患者社区护理管理中的应用效果评价

Application effect evaluation of family doctor contract service in community nursing management of elderly patients with hypertension

目的:探究家庭医生签约服务在老年高血压患者社区护理管理中的应用效果。方法:选取2019年2月-2020年3月社区老年高血压患者120例,随机分为两组,各60例。对照组实施常规社区护理管理;观察组实施家庭医生签约服务。比较两组干预效果。结果:观察组干预后患者自我管理能力评分、用药依从性评分、生活质量评分、对高血压知识了解度评分以及血压改善情况均显著优于对照组,差异有统计学意义(P<0.05)。结论:在社区护理管理中,家庭医生签约服务应用于老年高血压患者的效果显著,可以改善患者血压水平,提高患者生活质量。

Objective:To explore the application effect of family doctor contract service in community nursing management of elderly patients with hypertension.Methods:From February 2019 to March 2020,120 elderly patients with hypertension in the community were selected and randomly divided into two groups with 60 cases in each group.The control group was given routine community nursing management.The observation group was given family doctor contract service.We compared the intervention effect between the two groups.Results:In the observation group,after the intervention,the scores of self-management ability,medication compliance,quality of life,knowledge of hypertension and improvement of blood pressure in the observation group were significantly better than those in the control group,the difference was statistically significant(P<0.05).Conclusion:In community nursing management,the effect of family doctor contract service applied to elderly patients with hypertension is significant,it can improve the blood pressure level of patients and improve the quality of life of patients.
