复方苦参注射液治疗晚期原发性肝癌临床疗效及对血清Pokemon水平的影响

Clinical efficacy of treating advanced primary liver cancer with compound Kushen injection and its effect on Pokemon expression level

目的观察复方苦参注射液治疗晚期原发性肝癌的临床疗效及对血清Pokemon表达水平的影响。方法将40例晚期原发性肝癌患者按照随机数字表法分为2组。对照组19例予护肝、利尿、镇痛、纠正水电解质紊乱及酸碱平衡失调、营养支持等常规支持治疗,治疗组21例在对照组治疗基础上加用复方苦参注射液。比较2组实体瘤近期疗效、生活质量、总生存时间及疾病进展时间,以及治疗前后血清Pokemon水平,采用Cox多变量分析影响患者预后的因素。结果2组实体瘤近期疗效有效率比较差异无统计学意义(P>0.05),疗效相当。治疗组生活质量提高率76.19%(16/21),高于对照组26.32%(5/19)(P<0.05)。治疗组总生存时间及疾病进展时间长于对照组(P<0.05)。复方苦参注射液治疗、TNM分期为原发性肝癌患者预后的独立因素。治疗后治疗组Pokemon水平较本组治疗前降低(P<0.05),对照组较本组治疗前升高(P<0.05);治疗后治疗组血清Pokemon水平低于对照组(P<0.05)。结论复方苦参注射液可提高晚期原发性肝癌患者的生活质量,延长生存期及肿瘤进展时间,其机制可能与减少血清Pokemon的水平有关。

Objective To observe the clinical efficacy of treating advanced primary liver cancer with compound Kushen injection and explore its effect on serum Pokemon expression level.Methods Forty patients with advanced primary liver cancer were divided according to the random number table into the control group(n=19)with the conventional supportive treatment including liver protection,diuretic therapy,analgesia,correction of acid-base imbalance and electrolyte disorder and nutritional support and the control group(n=21)with compound Kushen injection on the basis of the treatment of the control group.The short-term effect of solid tumor,quality of life,overall survival time,time to progression,and Pokemon expression levels before and after the treatment between the two groups were analyzed.The multivariable Cox regression was applied to analyzed the factors that influenced patients'prognosis.Results There was no statistically significant difference in the short-term effect of solid tumor between the two groups(P>0.05),which means the comparative effect of two groups.The improvement rate of quality of life in the treatment group(76.19%,16/21)was higher than that of the control group(26.32%,5/19)(P<0.05).The overall survival time and time to progression in the treatment group were longer than those of the control group(P<0.05).The compound Kushen injection or TNM staging was an independent risk factor for the prognosis of patients.After the treatment,the Pokemon expression level in the treatment group was lower than before the treatment(P<0.05),while the level in the control group higher(P<0.05).After the treatment,the Pokemon expression level in the treatment group was lower than that of the control group(P<0.05).Conclusion Compound Kushen injection can improve the quality of life,prolong the overall survival time or time to progression in patients with advanced primary liver cancer,and its mechanism may be related to the decrease of serum Pokemon expression level.
