不同证候糖尿病肾病血清肾胺酶表达及其相关性研究 被引量:2

Expression and Correlation of Serum Renalase in Diabetic Nephropathy with Different Syndromes

摘要 目的观察不同证候糖尿病肾病(DN)血清肾胺酶(RNLS)表达及其相关性。方法选取DN患者120例,根据中医辨证分型分为阴虚燥热型组12例,气阴两虚型组26例,脾肾气虚型组37例,阴阳两虚型组45例;选取同期健康体检者106例作为健康对照组。检测比较各组血清RNLS水平及血尿素氮(BUN)、肌酐(Cr)水平,CKD-EPI公式估算肾小球滤过率(e GFR);采用Pearson法分析不同中医证候DN患者血清RNLS水平与BUN、Cr及e GFR的相关性。结果阴虚燥热组、气阴两虚组、脾肾气虚组、阴阳两虚组DN患者血清RNLS水平均显著高于健康对照组(P<0.05),且脾肾气虚组、阴阳两虚组血清RNLS水平均高于阴虚燥热组、气阴两虚组(P<0.05)。健康对照组、阴虚燥热组、气阴两虚组、脾肾气虚组、阴阳两虚组BUN、Cr水平依次升高(P<0.05),e GFR水平依次降低(P<0.05)。Pearson相关性分析结果显示,不同中医证候DN患者血清RNLS水平均与BUN、Cr呈正相关(P<0.05),与e GFR呈负相关(P<0.05)。结论血清RNLS水平在DN患者不同中医证候中存在差异性表达,且与DN患者BUN、Cr呈正相关,与e GFR呈负相关。 Objective To observe the expression and correlation of serum renalase( RNLS) in diabetic nephropathy( DN) with different syndromes. Methods A total of 120 DN patients were selected and divided into yin deficiency and dryness-heat group( 12 cases),qi and yin deficiency group( 26 cases),spleen and kidney qi deficiency group( 37 cases) and yin and yang deficiency group( 45 cases) according to TCM syndrome differentiation.106 healthy subjects were selected as the control group. The levels of serum RNLS,blood urea nitrogen( BUN) and creatinine( Cr) in each group were detected and compared,and the glomerular filtration rate( e GFR) was estimated by CKD-EPI formula. Pearson method was used to analyze the correlation between serum RNLS level and BUN,Cr and e GFR in DN patients with different TCM syndromes. Results The serum RNLS levels of DN patients in the yin deficiency and dryness-heat group,the qi-yin deficiency group,the spleen-kidney qi deficiency group and the yin-yang deficiency group were significantly higher than those in the control group( P < 0. 05),and the serum RNLS levels in the spleen-kidney qi deficiency group and the yin-yang deficiency group were higher than those in the yin deficiency and dryness-heat group and the qi-yin deficiency group( P < 0. 05). The levels of BUN and Cr in the control group,yin deficiency and dryness-heat group,qi-yin deficiency group,spleen-kidney qi deficiency group and yin-yang deficiency group increased in turn( P < 0. 05),and the level of e GFR decreased in turn( P <0. 05). Pearson correlation analysis showed that the serum RNLS levels in DN patients with different TCM syndromes were positively correlated with BUN and Cr( P < 0. 05),and negatively correlated with e GFR( P < 0. 05). Conclusion The serum RNLS level was differentially expressed in different TCM syndromes of DN patients,and was positively correlated with BUN and Cr of DN patients,and negatively correlated with e GFR.
