尿路结石患者取石术后复发的影响因素分析

Analysis of influence factors of postoperative recurrence in patients with urinary calculi after lithotomy

目的分析尿路结石患者取石术后复发的影响因素,为选择合理的防范措施提供参考。方法回顾性分析2019年1~12月广东省中山市板芙医院收治的150例行尿路结石取石术患者的临床资料,分析复发相关因素及独立危险因素。结果150例患者中复发22例,复发率为14.67%。单因素分析结果显示,不同体重指数、有无结石家族史、有无泌尿系感染、有无代谢异常、不同饮食习惯、不同结石数量的结石复发率比较,差异有统计学意义(P<0.05)。多因素Logistic回归分析结果显示,体重指数≥25 kg/m2、泌尿系感染、代谢异常、甜腻饮食习惯为结石复发的独立危险因素(P<0.05)。结论尿路结石患者取石术后需注重控制体重,养成良好的饮食习惯,少食甜腻,同时术后需接受规范化康复治疗,避免出现代谢紊乱、泌尿系感染等不良情况,以降低复发率。

Objective To analyze the influence factors of postoperative recurrence in patients with urinary calculi after lithotomy,and to provide the reference for choosing reasonable preventive measures.Methods Clinical data of 150 patients with recurrent urinary calculi who were treated in Banfu Hospital of Zhongshan City,Guangdong Province from January to December 2019 were retrospectively analyzed.The recurrence related factors and independent risk factors were analyzed.Results A total of 22 cases recurred in 150 patients,the recurrence rate was 14.67%.The results of single factor analysis showed that different body mass index,with or without family history of stones,urinary tract infection,metabolic abnormality,different dietary habits,number of stones were significantly correlated with stone recurrence rate,and the differences were statistically significant(P<0.05).Multivariate Logistic analysis showed that body mass index≥25 kg/m2,urinary tract infection,metabolic abnormality and sweet and greasy eating habits were independent risk factors for stone recurrence(P<0.05).Conclusion The patients with urinary calculi need to pay attention to weight control,develop good eating habits,with less sweet and greasy,and receive standardized rehabilitation treatment to avoid metabolic disorders,urinary tract infection and other adverse conditions,so as to reduce the recurrence rate.
