栓道式精密附着体义齿治疗后牙游离缺失的临床效果

Clinical effect of key-key way precision attachment denture in the treatment of posterior dental dissociation loss

目的探讨栓道式精密附着体义齿治疗后牙游离缺失的临床效果。方法选取2017年1月~2018年9月大连市第三人民医院收治的96例后牙游离缺失患者作为研究对象,采用随机数字表法将其分为对照组(48例)与研究组(48例)。对照组患者采用常规方法治疗,研究组患者采用栓道式精密附着体义齿治疗,两组均连续治疗2周。比较两组患者的牙周探诊深度、牙龈指数、固位力、咀嚼功能、义齿总满意率及修复失败情况。结果首次佩戴,两组患者的牙周探诊深度、牙龈指数、固位力及咀嚼功能比较,差异无统计学意义(P>0.05)。佩戴1年,两组患者的牙周探诊深度比较,差异无统计学意义(P>0.05)。佩戴1年,研究组患者的牙龈指数低于对照组,固位力、咀嚼功能高于对照组,差异有统计学意义(P<0.05)。两组患者佩戴1年的牙周探诊深度与首次佩戴比较,差异无统计学意义(P>0.05)。两组患者佩戴1年的牙龈指数、咀嚼功能高于首次佩戴,差异有统计学意义(P<0.05)。两组患者佩戴1年的固位力低于首次佩戴,差异有统计学意义(P<0.05)。研究组患者的总满意率高于对照组,差异有统计学意义(P<0.05)。研究组与对照组均无修复失败患者。结论后牙游离缺失治疗中,栓道式精密附着体义齿的效果明显,值得应用。

Objective To investigate the clinical effect of key-key way precision attachment denture in the treatment of posterior dental dissociation loss.Methods A total of 96 patients with posterior dental dissociation loss admitted to the Third People′s Hospital of Dalian City from January 2017 to September 2018 were selected as study subjects,and were divided into control group(48 cases)and study group(48 cases)by random number table method.Patients in the control group were treated with conventional method,while patients in the study group were treated with key-key way precision attachment denture.Both groups were treated for 2 weeks consecutively.The periodontal exploration depth,gingival index,retention force,masticatory function,satisfaction rate of denture,and repair failure were compared between the two groups.Results There were no significant differences in the periodontal exploration depth,gingival index,retention force,masticatory function between the two groups on the first wear(P>0.05).After wearing for 1 year,there was no statistically significant difference in the periodontal exploration depth between the two groups(P>0.05).After wearing for 1 year,gingival index of the study group was lower than that of the control group,retention force and masticatory function were higher than those of the control group,with statistically significant differences(P<0.05).There was no statistically significant difference between the two groups in the periodontal exploration depth after wearing for 1 year and on the first wear(P>0.05).The gingival index and masticatory function of the two groups after wearing for 1 year were higher than those on the first wear,with statistically significant differences(P<0.05).The retention force of patients in the two groups after wearing for 1 year was lower than that on the first wear,and the difference was statistically significant(P<0.05).The total satisfaction rate of the study group was higher than that of the control group,and the difference was statistically significant(P<0.05).There were no patients with repair failure in study group and control group.Conclusion In the treatment of posterior dental dissociation loss,the therapeutic effect of key-key way precision attachment denture is obvious,which is worth applying.
