摘要
目的探讨癌胚抗原(CEA)、糖类抗原(CA199)、糖类抗原125(CA125)、血管生成素(Ang-12)、细胞角蛋白19片段(CYFRA211)及甲胎蛋白(AFP)在肺癌诊断中的价值。方法选取2016年1月至2019年4月本院收治的肺癌患者85例(肺癌组)、肺部良性病变患者90例(肺部良性病变组),另选健康体检者80名(对照组)作为研究对象,肺癌组根据病理检查结果分为鳞癌组(n=41)、腺癌组(n=29)和小细胞癌组(n=15),比较3组CEA、CA199、CA125、Ang-2、CYFRA211及AFP水平。结果肺癌组CEA、CA199、CA125、Ang-2、CYFRA211及AFP水平均高于对照组与肺部良性病变组(P<0.05);小细胞癌组Ang-2水平明显低于腺癌组与鳞癌组(P<0.05);鳞癌组CYFRA211水平明显高于小细胞癌组与腺癌组(P<0.05);不同病理分期肺癌患者的各项指标比较差异均有统计学意义(P<0.05)。结论CEA、CA199、CA125、Ang-2、CYFRA211及AFP在肺癌诊断中的应用效果较好,对肺癌病理分期与组织学分型判断具有一定参考价值,值得临床推广应用。
Objective To investigate the diagnostic value of carcinoembryonic antigen(CEA),carbohydrate antigen(CA199),carbohydrate antigen 125(CA125),angiopoietin(Ang-12),Cytokeratin-19-fragment(CYFRA211)and alpha fetoprotein(AFP)for lung cancer.Methods 85 lung cancer patients(lung cancer group),90 patients with benign lung diseas(benign lung disease group)and 80 people(control group)controls who were admitted to our hospital from January 2016 to April 2019 were selected as the research subjects,the lung cancer group was divided into squamous cell carcinoma group(n=41),adenocarcinoma group(n=29)and small cell carcinoma group(n=15)according to the pathological examination results.The expression of CEA,CA199,CA125,Ang-2,CYFRA211 and AFP were compared among the three groups.Results The levels of CEA,CA199,CA125,Ang-2,CYFRA211 and AFP in lung cancer group were higher than in benign lung disease group and control group(P<0.05);the level of Ang-2 in small cell carcinoma group was lower than adenocarcinoma group and squamous cell carcinoma group(P<0.05);the level of CYFRA211 in squamous cell carcinoma group was higher than that in adenocarcinoma group and squamous cell carcinoma group(P<0.05).There was significant difference in the indexes of lung cancer patients with different pathological stages(P<0.05).Conclusion CEA,CA199,CA125,Ang-2,CYFRA211 and AFP have good diagnostic efficacy for lung cancer,which have certain reference value for the diagnosis of lung cancer pathological staging and histological classification,and are worthy of clinical promotion and application.
