肺炎支原体23Sr RNA耐药基因位点突变情况与儿童呼吸道感染患者临床症状分析 被引量:3

摘要 目的通过对儿童进行肺炎支原体核酸及耐药基因突变23Sr RNA基因位点聚合酶链式反应检测,分析患儿肺炎支原体核酸检测阳性及耐药基因阳性的分布特点及其与临床症状的相关性。方法 896例检测标本收集于焦作市妇幼保健院儿科一区(呼吸病区),时间2019年1月1日至2019年8月1日,采用聚合酶链式反应技术结合TaqMan荧光探针,对儿童呼吸道感染患者进行鼻咽拭子取样检测。针对23Sr RNA基因突变位点设计引物探针,检测肺炎支原体耐药基因突变位点A2603G、A2604G突变情况,并对肺外感染、住院时长、发热时长等临床症状进行分析。结果 896例儿童呼吸道感染患者中,肺炎支原体(MP)核酸检测阳性98例,阳性率10.9%。其中耐药基因突变位点A2603G和A2604G突变83例,检测阳性占MP阳性患者84.7%。突变组肺外感染、住院时长、发热时长均显著高于非突变组。结论肺炎支原体23Sr RNA耐药基因在儿童呼吸道感染患者中,随年龄增加,A2603G和A2604G位点突变率增高;儿童呼吸道感染患者23Sr RNA基因位点A2603G和A2604G突变阳性,易发生肺炎和肺外感染。 Objective To detect the 23Sr RNA gene mutation in mycoplasma pneumoniae DNA and drug resistance gene in children by polymerase chain reaction,the distribution of mycoplasma pneumoniae nucleic acid test positive and drug resistance gene positive in children and their correlation with clinical symptoms were analyze.Methods 896 specimens were collected from pediatrics area 1 of Jiaozuo maternal and child health hospital from January 1,2019 to August 1,2019,using TaqMan fluorescent probe and polymerase chain reaction technique,nasopharyngeal swabs were taken in children with respiratory tract infection.To detect the mutations of A2603G and A2604G in mycoplasma pneumoniae resistance gene,we designed a primer probe for 23Sr RNA gene mutation,and analyzed the clinical symptoms such as extrapulmonary infection,length of hospitalization and length of fever.Results Of the 896 children with respiratory tract infection,98 cases(10.9%)were positive for mycoplasma pneumoniae.Among them,83 cases with mutations of A2603G and A2604G were positive for MP(84.7%).The time of extrapulmonary infection,hospitalization and fever in mutation group were significantly higher than that in non-mutation group.Conclusion The mutation rates of A2603G and A2604G in mycoplasma pneumoniae 23Sr RNA resistance gene were increased with age in children with respiratory tract infection,and the mutation rates of A2603G and A2604G in children with respiratory tract infection were positive,prone to pneumonia and extrapulmonary infection.
