摘要
目的探讨小儿胃食管反流病(GERD)与胃食管阀瓣分级的相关性,为临床早期诊治提供重要参考。方法回顾性分析郑州大学附属儿童医院2019年1月至2019年12月收治的97例GERD患儿的临床资料,所有患者均给予胃镜检查,按照胃食管阀瓣分级将其分为Ⅰ级组(n=24)、Ⅱ级组(n=20)、Ⅲ级组(n=39)、Ⅳ级组(n=14),并采用GERD症状评估量表评估患儿严重程度,对比四组GERD症状评分,并分析胃食管阀瓣分级与GERD的相关性。结果四组中,Ⅳ级组GERD症状评分最高,其次是Ⅲ级组、Ⅱ级组,Ⅰ级组最低(P<0.05);四组患儿GERD症状严重程度(轻度、重度、极重度)检出情况对比,差异明显(P<0.05);经等级资料Spearman相关性检验证实,小儿胃食管阀瓣分级与GERD的症状严重程度呈正相关(r=0.523,P=0.000)。结论小儿胃食管阀瓣分级与GERD的症状严重程度呈正相关,即胃食管阀瓣分级越高,患儿疾病症状越严重。
Objective To investigate the relationship between the grading of gastroesophageal valve disc and acid reflux disease in children,so as to provide important reference for clinical early diagnosis and treatment.Methods The clinical data of 97 children with GERD admitted to children's hospital of Zhengzhou University from January 2019 to December 2019 were analyzed retrospectively,according to the classification of gastroesophageal valve flap,the patients were divided into three groups:gradeⅠgroup(N=24),gradeⅡgroup(N=20),gradeⅢgroup(N=39)and gradeⅣgroup(N=14).The severity of GERD was assessed with GERD symptom evaluation scale,the correlation between GERD and the grading of gastroesophageal valve disc was analyzed.Results Among the four groups,gradeⅣgroup had the highest GERD score,followed by gradeⅢgroup and gradeⅡgroup,and gradeⅠgroup had the lowest GERD score(P<0.05).The results of spearman correlation test showed that there was a positive correlation between the grades of gastroesophageal valve and the severity of GERD(r=0.523,P=0.000).Conclusion There is a positive correlation between GERD and gastroesophageal valve disc grading,the higher the gastroesophageal valve disc grading,the more severe the disease symptoms in children.
