头穴透刺治疗急性中风病的针刺时机对患者疗效及神经恢复的影响研究

A study on efficacy of acupuncture timing of scalp acupuncture on acute stroke and nerve function recovery

目的:探究头穴透刺治疗的时机选择对急性中风的疗效的影响。方法:选取2018年5月-2019年5月我院收治的45例急性中风病患者作为研究对象,均采取头穴透刺治疗,根据治疗时机的不同进行分组。对照组患者于后遗症期接受针刺治疗,观察组患者于急性期接受针刺治疗,评估两组治疗总效果,记录评估各组患者入院时及治疗后证候表现,比对差异情况,另外对比两组患者治疗后神经缺损情况及偏瘫运动功能、生活能力。结果:观察组患者于急性期及时治疗后效果明显优于对照组(P <0.05)。两组患者入院时相关证候评分无显著差异(P> 0.05),治疗后均得到改善,观察组患者面瘫证候评分、失语证候评分、吞咽障碍证候评分、感觉障碍评分、意识障碍评分均较对照组更低(P <0.05)。另外,治疗后观察组患者神经功能缺损程度量表评分(Neurological Deficit Score,NDS)低于对照组,偏瘫Fugl-Meyer运动功能评估量表评分(Fugl-Meyer Assessment Scale,FMAS)、生活质量量表评分(Quality of Life Score,QOL)较对照组更高(P <0.05)。观察组改良Ash-worth肌张力评分显著低于对照组,差异有统计学意义(P <0.05)。结论:急性中风病患者行头穴透刺治疗效果显著,且针刺时间越早疗效越佳,有助于改善患者症状,提高生活能力。

Objective: To investigate the effects of acupuncture timing of scalp acupuncture on acute stroke. Methods: 45 cases in our hospital from May 2018 to May 2019 were selected. All the patients were treated by scalp acupuncture and grouped according to the timing of treatment. Patients in the control group received acupuncture during the sequelae period. Patients in the observation group received acupuncture in the acute phase. Results: Clinical effects in the observation group were significantly better than the control group after timely treatment(P<0.05). There was no significant difference in the related symptom scores at admission between the groups(P>0.05), and the related symptom scores in groups were both improved after treatment. In the observation group, facial paralysis score, aphasia syndrome score, dysphagia syndrome scroe, sensory disturbance score and consciousness disturbance score were all lower than those in the control group(P<0.05). In addition, the NDS after treatment in the observation group was lower than the control group(P<0.05), and FMAS and QOL were higher than those in the control group(P<0.05). The modified Ash-worth muscle tone score in the observation group was significantly lower than the control group, P<0.05. Conclusion: The effects of scalp acupuncture on acute stroke are remarkable, and the earlier acupuncture time, the better the effects, which help to improve the symptoms of patients and their living ability.
