孟鲁司特钠合并手术对腺样体肥大鼻窦炎患儿的疗效观察

Therapeutic Effect of Montelukast Combined with Surgical Resection Therapy on Sinusitis Complicated with Adenoidal Hypertrophy in Children

目的探讨孟鲁司特钠联合手术治疗学龄前儿童腺样体肥大合并鼻窦炎的疗效。方法将2016年1月至2018年12月收治的腺样体肥大并发慢性鼻-鼻窦炎的学龄前儿童88例,根据治疗方法不同分为内科组(n=30)、手术组(n=30)、合并组(n=28)。3组均给予鼻用激素治疗。内科组采用孟鲁司特钠药物治疗,手术组采用手术切除治疗,合并组采用孟鲁司特钠联合手术治疗。治疗后随访2个月,评价各治疗组不同时间段患儿的疗效差异。结果在治疗第2、6、12周时合并组患儿疗效明显优于内科组(P<0.05);在治疗第2、12周时手术组与合并组患儿疗效接近(P>0.05);在治疗第6周时合并组患儿疗效明显优于单纯手术组(P<0.05)。结论孟鲁司特纳联合手术治疗,可缩短学龄前儿童腺样体肥大合并慢性鼻-鼻窦炎的疗程,提高治愈率,对患儿症状的早期恢复具有重要的促进作用。

Objective To explore the therapeutic effect of montelukast combined with surgical resection therapy on adenoidal hypertrophy complicated with sinusitis in preschool children.Methods A total of 88 preschoolers with adenoidal hypertrophy complicated with chronic sinusitis treated between January 2016 and December 2018 were selected and divided into the drug therapy group(n=30),the operation group(n=30),and the combined treatment group(n=28).All the three groups were given the nasal hormone therapy.The drug therapy group was treated with montelukast,the operation group was treated with surgical resection,and the combined treatment group was treated with montelukast combined with surgical resection.The therapeutic effect at different time points were evaluated and compared after 2 months of follow-ups among the three groups.Results The therapeutic effect of the combined treatment group was significantly better than that of the drug therapy group at the 2nd,6th and 12th week respectively(P<0.05),and that of the operation group was similar to that of the combined treatment group at the 2nd and 12th week respectively(P>0.05).The therapeutic effect of the combined treatment group was significantly better than that of the operation group(P<0.05).Conclusion Montelukast combined with surgical resection therapy can shorten the course of treatment and improve the cure rate in the treatment of adenoidal hypertrophy with chronic sinusitis in preschool children,which plays an important role in their early recovery.
