不同止血方式对腹腔镜下卵巢囊肿剥除术患者性激素水平及卵巢功能的影响

Effect of different hemostatic methods on sex hormone levels and ovarian function in patients with laparoscopic ovarian cystectomy

目的:探讨不同止血方式对腹腔镜下卵巢囊肿剥除术患者性激素水平及卵巢功能的影响。方法:选取某院治疗的78例卵巢囊肿患者,将其分为对照组与观察组,每组39例。2组均行腹腔镜下卵巢囊肿剥除术,对照组采用电凝止血,观察组采用可吸收线“8”字缝合止血。比较2组患者性激素水平〔雌激素(E2)、孕激素(P)、卵泡刺激素(FSH)、黄体生成激素(LH)、睾酮(T)〕、血流动力学〔心率(HR)、平均动脉压(MAP)、心输出量(CO)〕及卵巢功能〔卵巢间质血流收缩期峰值(PSV)、卵巢窦卵泡数〕情况。结果:术后1周,观察组E2、T水平高于对照组,P、FSH、LH水平低于对照组,差异有统计学意义(P<0.05);术后1周,观察组HR、MAP、CO低于对照组,差异有统计学意义(P<0.05);术后3个月,观察组PSV、卵巢窦卵泡数高于对照组,差异有统计学意义(P<0.05)。结论:缝合止血方式在卵巢囊肿切除术中的应用利于促进患者术后性激素水平及卵巢功能的恢复,可减小血液流变学的改变,维持PSV及卵巢窦卵泡数的正常数值。

Objective:To investigate the effect of different hemostatic methods on sex hormone levels and ovarian function in patients with laparoscopic ovarian cystectomy.Methods:78 patients with ovarian cyst treated in a hospital were selected and randomly divided into a control group and an observation group,with 39 cases in each.The two groups underwent laparoscopic ovarian cystectomy.After cyst dissection,the control group was given electric coagulation hemostasis,and the observation group was treated with absorbable thread"8"suture without electric coagulation hemostasis.The sex hormone levels,hemodynamics and recovery of menstruation were compared between the two groups.Results:One week after the surgery,the levels of E2 and T in the observation group were higher than those in the control group,and the levels of P,FSH and LH were lower than those in the control group(P<0.05).One week after the surgery,HR,MAP and CO in the observation group were lower than those in the control group(P<0.05).Three months after the surgery,PSV and ovarian antral follicle count in the observation group were higher than those in the control group(P<0.05).Conclusion:Suture hemostasis in ovarian cystectomy can promote recovery of sex hormone levels,reduce the changes in hemorheology,and maintain the normal values of PSV and ovarian antral follicle count,thus protecting the ovarian function.
