儿科应用奥美拉唑肠溶片的风险及对策

Risk and Countermeasures of Application of Omeprazole Enteric-Coated Tablets in Pediatrics

目的:对奥美拉唑肠溶片在儿科应用的合理性进行分析,为儿科临床合理使用奥美拉唑肠溶片提供参考。方法:参照质子泵抑制剂(PPIs)成人用药指南、有关文献及药品说明书,总结2016-2017年自贡市第一人民医院儿科使用奥美拉唑肠溶片的不合理性并提出对策。结果:2016-2017年该院儿科使用奥美拉唑肠溶片不合理87例(67.97%),其中无适应证用药62例(71.26%),用药剂量不合理25例(28.74%)。结论:奥美拉唑肠溶片用药不合理率高,导致治疗风险大。诱因有盲目用药、用药不准确及药师参与不够。临床用药应明确适应证、合理选用药物;明确给药剂量、确保用药安全有效;药师参与指导、提升用药合理性。

Objective:To analyse the application rationality of omeprazole enteric-coated tablets in pediatrics,so as to provide reference for the rational drug use in clinic.Methods:With reference to the guidelines for adult use of proton mercury inhibitors(PPIs),relevant literature and drug instructions,the application irrationality of omeprazole enteric-coated tablets in pediatrics of Zigong First People’s Hospital from 2016 to 2017 was summarized and the countermeasures were proposed.Results:From 2016 to 2017,there were 87 cases(67.97%)of irrational application of omeprazole enteric-coated tablets in pediatrics of our hospital,among which 62 cases(71.26%)had no indications,and 25 cases(28.74%)had irrational dosage.Conclusion:The irrational application rate of omeprazole enteric-coated tablets is relatively high,which leads to high treatment risks.The triggers are blind medication,inaccurate medication and insufficient participation of clinical pharmacists.In clinical medication,the indications should be clear,and the drugs should be selected rationally,the dosage should be clear to ensure the safety and effectiveness of medication,and the pharmacists are supposed to participate into the guidance and improve the rationality of medication.
