热敏灸联合中药离子导入对膝关节炎并关节腔积液疗效及炎性因子的影响

Effect of Heat Sensitive Moxibustion Combined with Iontophoresis of Traditional Chinese Medicine on Joint Effusion and Inflammatory Factors for Knee Arthritis

目的:观察热敏灸联合中药离子导入对膝关节炎并关节腔积液疗效及炎性因子的影响。方法:选取我院针灸科2016年6月~2019年6月收治膝关节骨性关节炎并关节腔积液患者120例作为研究对象,随机分为观察组与对照组各60例,对照组采用常规西药治疗,观察组在对照组基础上施以热敏灸联合中药离子导入治疗,观察患者临床疗效及炎性因子指标。结果:①治疗后,两组患者VAS评分及Lysholm评分均较治疗前显著改善(P<0.05),组间比较,观察组VAS评分及Lysholm评分显著优于对照组(P<0.05);②治疗后,两组患者WOMAC评分均较治疗前显著改善(P<0.05),组间比较,观察组WOMAC评分显著优于对照组(P<0.05);③对照组总有效率71.67%,观察组总有效率93.33%,观察组总有效率显著优于对照组(P<0.05);④治疗后,两组患者IL-1、TNF-α及MMP-3均较治疗前显著改善(P<0.05),且观察组IL-1、TNF-α及MMP-3显著优于对照组(P<0.05)。结论:热敏灸联合中药离子导入治疗膝关节骨性关节炎合并关节腔积液能够有效调节其炎性因子表达,减轻患者临床症状,促进功能恢复,临床疗效显著,值得推广。

Objective:To investigate the effect of heat sensitive moxibustion combined with iontophoresis of traditional Chinese medicine on knee arthritis with effusion in articular cavity and the influence of inflammatory factors.Methods:120 patients with knee osteoarthritis complicated with effusion in the acupuncture and moxibustion department of our hospital from June 2016 to June 2019 were selected and randomly divided into observation group and controlled group,with 60 cases in each group.The controlled group was treated with conventional western medicine,while the observation group was given heat sensitive moxibustion combined with iontophoresis of traditional Chinese medicine on the basis of the controlled group.Results:①After treatment,the VAS score and Lysholm score of the two groups were significantly improved(P<0.05),the VAS score and Lysholm score of the observation group were significantly better than those of the controlled group(P<0.05);②After treatment,the WOMAC scores of the two groups were significantly improved(P<0.05),and the WOMAC score of the observation group was significantly better than that of the controlled group(P<0.05);③The total effective rate of the observation group was significantly better than that of the controlled group(P<0.05),IL-1,TN F-α and MMP-3 in the observation group were significantly better than those in the controlled group(P<0.05).Conclusion:Heat sensitive moxibustion combined with iontophoresis of traditional Chinese medicine can effectively regulate the expression of inflammatory factors,reduce the clinical symptoms and promote the functional recovery of patients with knee osteoarthritis and joint effusion.
