针灸联合中药离子导入治疗卒中后肩手综合征临床研究

Clinical Study of Applying Acupuncture Combined with Iontophoresis of Traditional Chinese Medicine in the Treatment of Shoulder Hand Syndrome after Stroke

目的:观察分析针灸联合中药离子导入治疗卒中后肩手综合征的临床疗效。方法:选取本院2019年3月至2020年5月收治的82例卒中后肩手综合征患者作为研究对象,通过数字奇偶法分为对照组(n=41)和观察组(n=41),在常规西医治疗的基础上对照组加以针灸进行治疗,观察组加以针灸联合中药离子导入进行治疗,观察比较两组患者临床疗效、肩关节疼痛与手肿程度、神经功能以及肩关节活动功能。结果:临床总疗效对比,对照组患者的临床总疗效为78.05%,观察组患者的临床总疗效为95.12%,观察组临床疗效高于对照组(P<0.05);肩关节疼痛与手肿程度对比,治疗后两组患者VAS评分与上肢容积均较治疗前降低,且观察组低于对照组(P<0.05);神经功能对比,治疗后两组患者MESSS、NIHSS神经功能评分均较治疗前降低,且观察组低于对照组(P<0.05);肩关节活动功能对比,治疗后两组患者Fugl-Meyer上肢运动功能评分均优于治疗前,且观察组优于对照组(P<0.05)。结论:针灸联合中药离子导入治疗卒中后肩手综合征患者,其临床效果更佳,更能缓解患者肩关节疼痛与手肿程度,改善神经功能,促进肩关节活动功能恢复。

Objective:To explore and analyze the clinical research of acupuncture combined with iontophoresis of traditional Chinese medicine in the treatment of shoulder hand syndrome after stroke.Methods:82 cases of post-stroke shoulder hand syndrome patients in our hospital from March 2019 to may 2020 were selected as the research objects,and were divided into a controlled group(n=41)and an observation group(n=41)by digital parity method.On the basis of conventional western medicine treatment,the controlled group was treated with acupuncture and moxibustion combined with iontophoresis of traditional Chinese medicine.The clinical efficacy and shoulder relationship of the two groups were observed and compared joint pain and hand swelling degree,nerve function and shoulder joint activity function.Results:Comparison of total clinical efficacy,the total clinical efficacy of the controlled group was 78.05%,and that of the observation group was 95.12%.The clinical efficacy of the observation group was higher than that of the controlled group(P<0.05).The degree of shoulder pain and hand swelling was compared,after treatment,VAS score and upper limb volume of the two groups were lower than those before treatment,and the observation group was lower than the controlled group(P<0.05).Comparison of neurological function,after treatment,the scores of mESSS and NIHSS in the two groups were lower than those before treatment,and the observation group was lower than the controlled group(P<0.05).Comparison of shoulder joint activity function,after treatment,Fugl Meyer motor function scores of the two groups were better than before treatment,and the observation group was better than the controlled group(P<0.05).Conclusion:Applying acupuncture combined with traditional Chinese medicine iontophoresis in the treatment of post-stroke shoulder hand syndrome patients,its clinical effect is better,more can alleviate the shoulder joint pain and hand swelling,improve nerve function,promote the recovery of shoulder joint activity function.
