皮肤减张器在整形外科张力性手术切口中的应用

Application of Skin Tension Reducing Suture Device in Tension Incision of Plastic Surgery

目的:探讨皮肤减张器对整形外科张力性手术切口愈合及后期瘢痕增生的影响。方法:2018年6月-2019年6月笔者科室共收治各类张力性手术切口患者110例,通过皮下逐层减张缝合、皮内间断缝合、皮肤美容缝合、皮肤减张器外固定的方法,从皮下、皮内及皮肤表面三个层次来充分减张,使切口低张力愈合,观察切口愈合情况及瘢痕增生情况。结果:110例张力性手术切口均一期愈合,术后均未发生感染、红肿及裂开。随访6个月,除2例面部、2例腹部、2例胸部、1例前臂、1例大腿、2例腹股沟区、1例背部瘢痕稍明显外,其余99例瘢痕纤细不明显,4例瘢痕疙瘩切口,均无复发。2例减张缝合器固定部位出现水疱,经处理后痊愈。结论:皮肤减张器能促进整形外科张力性手术切口愈合,减轻瘢痕增生。

Objective To investigate the effect of skin tension reducing suture device on wound healing and scar hyperplasia in plastic surgery.Methods From June 2018 to June 2019,the author treated were department of all kinds of tension incision of 110 cases of patients,through the subcutaneous layer by tension-reduced suture,intradermal interrupted suture,skin beauty suture,skin graft loads external fixation method of reduction from the skin,skin and skin surface three levels to fully cut piece,make low tension of incision healing,to observe the healing of incision and scar hyperplasia.Results All the 110 cases of tension surgical incisions healed in the first stage,and no infection,swelling or split occurred after the operation.During the follow-up for 6 months,except 2 cases of facial,2 cases of abdominal,2 cases of chest,one case of forearm,one case of ham,2 cases of inguinal area,one case back scar slightly obviously,the rest of the 99 cases of scar fine is not obvious,4 cases of keloid incision,no recurrence.Blisters appeared in the fixed site of 2 cases of skin tension reducing suture device,and recovered after treatment.Conclusion The skin tension reducing suture device can promote the wound healing and reduce the scar hyperplasia.
