肺炎支原体感染与儿童过敏性紫癜关系探讨

Clinical relevance between the infection of Mycoplasma pneumoniae and henoch-schonlein purpura in children

目的:探讨肺炎支原体感染与儿童过敏性紫癜发病的关系。方法:选取过敏性紫癜患儿56例(病例组)和同期体检正常的健康患儿30例(对照组)为研究对象,比较两组的MP-IgM阳性检出率。根据MP-IgM检测结果将病例组患儿分为MP感染组(MP-IgM阳性)、非MP感染组(MP-IgM阴性),比较两组的临床分型、测定病例组患儿的血清CD3^+、CD+4、CD8^+、CD4^+/CD8^+的表达水平。结果:①病例组肺炎支原体感染率明显高于健康儿童对照组(P<0.05);②病例组中MP感染组与非MP感染组临床分型比较差异有统计学意义(P<0.05),病例组中肺炎支原体感染肾脏损害所占比率(54.5%)高于非感染组(17.6%);③病例组中MP感染组CD3^+、CD4^+、CD4^+/CD8^+明显低于非MP感染组(均P<0.05)。结论:①肺炎支原体感染与过敏性紫癜发病有关;②MP感染与过敏性紫癜患儿临床分型有关,以肾型为主;③MP感染与过敏性紫癜免疫功能有关,可能会导致过敏性紫癜患儿的细胞免疫功能降低。

Objective:To analyze the correlation between Mycoplasma pneumoniae infection and henoch-schonlein purpura(HSP)in children.Methods:56 children with HSP(observation group)and 30 healthy children(control group)were selected as the research objects.The positive rate of MP-IgM was compared between the two groups.According to the MP-IgM detection results,the children in observation group were divided into MP-infected group(MP-IgM positive)and non-MP-infected group(MP-IgM negative),and the clinical types of the two groups were compared,and the expression levels of serum CD3^+、CD4^+、CD8^+、CD4^+/CD8^+in the observation group were determined.Results:The infection rate of Mycoplasma pneumoniae in the observation group was higher than that in the control group(P<0.05).There was statistically significant difference in clinical type between the two subgroups of the observation group(P<0.05),and the proportion of renal type in MP-infected group was higher than that in non-MP-infected group(P<0.05).CD3^+、CD4^+、CD8^+、CD4^+/CD8^+in MP-infected group were significantly lower than those in non-MP-infected group(all P<0.05).Conclusion:Mycoplasma pneumoniae infection is related to the pathogenesis of HSP.MP infection is associated with clinical type in children with HSP,mainly kidney type.MP infection is related to immune function of children with HSP,which may lead to the reduction of cellular immune function in children with HSP.
