分析灌洗液不同检测方法在支气管结核诊断中的价值

摘要 目的:分析在支气管结核诊断中不同灌洗液检测方法的价值。方法:研究对象是某院2017年11月~2018年11月期间收治的56例肺结核患者(观察组)与28例非结核性肺部疾病患者(对照组),均实施支气管镜检查,取肺泡灌洗液进行荧光定量PCR法、培养法、涂片法、结核抗体法检验结核杆菌,对比4种检验结果。结果:菌阳肺结核组FQ-PCR法的阳性率为89.29%,明显高于结核抗体法的50%、涂片法的14.29%,差异呈统计学意义(P<0.05);菌阴肺结核组的阳性率为82.14%,明显高于结核抗体法的32.14%,差异呈统计学意义(P<0.05);对照组与菌阴肺结核组对比,FQ-PCR法阳性率差异显著,呈现统计学意义(P<0.05)。结论:在支气管结核诊断中,荧光定量PCR检测法有着较高的诊断准确率、便捷、速度快。 Objective:To analyze the value of different lavage fluid detection methods in the diagnosis of bronchial tuberculosis.Methods:The subjects of this study were 56 patients with pulmonary tuberculosis(observation group)and 28 patients with non tuberculous pulmonary diseases(control group)who were admitted to a hospital from November 2017 to November 2018.All patients were examined by bronchoscopy.The bronchoalveolar lavage fluid was taken for fluorescence quantitative PCR,culture,smear and tuberculosis antibody methods to test for Mycobacterium tuberculosis,and four test results were compared.Results:In the bacterial positive tuberculosis group,the positive rate of FQ-PCR was 89.29%,which was significantly higher than 50%of TB antibody method and 14.29%of smear method.The difference was statistically significant(P<0.05).In the bacterial negative tuberculosis group,the positive rate of was 82.14%,which was significantly higher than 32.14%of TB antibody method,and the difference was statistically significant(P<0.05).The positive rate of FQ-PCR was significantly different between the control group and bacterial negative tuberculosis group,and the difference was statistically significant(P<0.05).Conclusion:In the diagnosis of bronchial tuberculosis,fluorescence quantitative PCR has high accuracy,convenience and speed.
