术前牙科焦虑症对口腔种植手术相关疼痛及满意度的影响

The influence of preoperative dental anxiety on perioperative pain and patient satisfaction of dental implant surgery

目的:调查口腔种植患者人群牙科焦虑的发生情况,分析患者术前的牙科焦虑对口腔种植术中疼痛及手术满意度的影响。方法:通过问卷调查的方式对某段时期在上海地区某三甲医院接受口腔种植手术的全部患者进行知情同意后开展调查,收集调查对象的人口学信息、术前牙科焦虑评分、术中疼痛评分以及对手术的满意度评分等资料进行统计描述与数据分析。结果:共完成调查486人,其中63.2%的调查对象术前存在轻度以上的牙科焦虑症,31.5%的调查对象术前存在中度或重度的牙科焦虑症;Spearman相关分析结果提示口腔种植患者术前的牙科焦虑状况与口腔种植的术中疼痛感存在显著的正相关(r=0.131, P<0.01),术前牙科焦虑评分和患者对口腔种植手术满意度的评分呈显著的负相关(r=-0.167, P<0.01);单因素和多因素分析结果均显示术前牙科焦虑症是口腔种植术中疼痛的显著影响因素(P<0.05);术前存在牙科焦虑症的患者对口腔种植手术的满意度也显著低于没有术前牙科焦虑症的患者(P<0.01)。结论:术前牙科焦虑对口腔种植患者的术中疼痛和手术满意度均有显著的影响,在临床工作中应予以重视并加强干预。

Objective:The aim of this study was to know the proportion of dental anxiety(DA)in patients receiving oral implant surgery,and to analyze the impact of DA on pain perception of patients during operation and their satisfaction level to the surgery.Methods:An investigational study was conducted in Grade A tertiary hospital in Shanghai.Consecutive patients receiving dental implant surgery were investigated after they signed informed consent.Information including demographic characteristics of patients,preoperative DA,intraoperative pain perception and patient satisfaction to dental implant surgery and so on was collected for statistical description and data analysis.Result:A total of 486 participants were investigated,63.2%of them had preoperative DA,31.5%of them had moderate or severe preoperative DA.The result of correlation analysis indicated that preoperative DA of patients correlated with pain perception during surgery(r=0.131,P<0.01)and scores of preoperative DA was negatively correlated with patient satisfaction to dental implant surgery(r=-0.167,P<0.01).Results of single factor and multiple factor analysis showed that preoperative DA had significant influence on intraoperative pain perception.Besides,it was found that patients with preoperative DA were less satisfied with the surgical treatment than those without preoperative DA(P<0.05).Conclusion:Preoperative DA was prevalent in patients with dental implant surgery.Intraoperative pain perception of patients can be strengthened by preoperative DA and degree of patient satisfaction could be negatively affected by preoperative DA.Thus,adequate attention should be paid to preoperative DA of patients with dental implant surgery.
