牙弓牙槽弓新分区在全牙弓种植修复的初步应用

摘要 目的:对牙弓牙槽弓新分区在临床全牙弓种植修复中的初步应用进行总结,分析新分区在全牙弓种植修复中的应用价值。方法:2017年3月-2019年12月,选取拟采用全牙弓种植修复的患者11例,在弓顶区、弓肩区、弓体区牙弓牙槽弓分区指导下,进行种植体植入位置设计,共完成16副全牙弓种植修复体,分析种植体植入数量、植入位点与分区的对应性,于即刻临时修复后1周、永久修复后半年,分别就临时修复体与永久修复体的满意度进行视觉量表法调查,对永久修复后即刻与6个月时种植体周围牙槽骨高度分别进行测量。结果:11例患者共植入牙种植体89枚,4枚、5枚、6枚和8枚种植体支持的全牙弓修复体分别为4副、3副、7副和2副,种植体植入位点分布与新分区对应性良好,患者对临时修复与永久修复均较为满意,满意度分别为4.62±0.13,4.81±0.08(P>0.05),永久修复完成后6个月,种植体周围牙槽骨高度未出现明显改变。结论:牙弓牙槽弓新分区在全牙弓种植修复中,能结合牙槽骨解剖条件,有效指导临床种植体植入数量、位点的设计与选择,为临床种植方案的设计、种植手术与修复的顺利实施提供了便利。 Objective:To analyze the application value of novel zone division of dental and alveolar arch based on summary of the preliminary application of the division in full-arch dental implantation.Methods:From 2017 March to 2019 December,11 cases were selected in the clinical trial with WEGO implant system.Sixteen full-arch implant supported prostheses were finished under the design guided by the novel zone division of dental and alveolar arch at the implant number choice and implantation sites decision.After restoration,the relationship of the design of implant number,the implant sites and the zone division was analyzed.The satisfaction of temporary restoration and permanent restoration were carried out respectively after one week and half a year.The height of bone level around implants was measured respectively immediately after permanent restoration and 6 months later.Results:89 implants were implanted in 11 cases and 16 full-arch dental implant supported prostheses were finished smoothly under the guidance of novel zone division of dental and alveolar arch.All cases were satisfied with the temporary restoration and the permanent restoration,the satisfaction score was 4.62±0.13,4.81±0.08(P>0.05)respectively.The prostheses supported by 4,5,6 and 8 implants were 4,3,7,2 respectively and the implant sites met the different zone of alveolar arch properly.No change of bone level around implants was found.Conclusions:The number of implants and the sites of implantation could be designed conveniently under the guidance of novel zone division of dental and alveolar arch.
