CLIA法检测全血γ-干扰素对活动性结核病的诊断价值

摘要 目的探究化学发光免疫分析法(CLIA法)检测全血γ-干扰素辅助诊断活动性结核病的临床价值。方法选择2019年3—12月在医院就诊的110例活动性结核病患者设为结核组,另选同期80例非结核性的肺部疾病患者设为非结核组,并选择同期80例健康体检者设为正常组,三组均采用化学发光免疫分析法检测全血γ-干扰素水平,比较其检测结果差异,并描绘ROC曲线评价该检测方法诊断结核病的效能。结果结核组的γ-干扰素水平较非结核组、正常组显著升高(P<0.05),且非结核组与正常组的γ-干扰素水平的差异无统计学意义(P>0.05)。以结核组为试验组,以非结核组、正常组为对照组,根据灵敏度和特异度之和最大的方法确定诊断临界值为218.92 pg/mL时,计算化学发光免疫分析法对活动性结核病诊断的灵敏度95.00%,特异度82.70%。结论化学发光免疫分析法检测全血γ-干扰素诊断活动性结核病的灵敏度高,有望成为本病辅助诊断的主要指标之一。 Objective To explore the clinical value of CLIA method in detecting whole blood gamma-interferon as an aid in the diagnosis of active tuberculosis.Methods From March to December 2019,110 patients with active tuberculosis in our hospital were selected as the tuberculosis group,80 patients with non-tuberculous pulmonary disease were selected as the non-tuberculous group,80 healthy checkups in the same period as the normal group,the three groups were detected by CLIA method for whole bloodγ-interferon levels,compared the test results.Results Theγ-interferon levels in the tuberculosis group were significantly higher than non-tuberculosis group and the normal group(P<0.05).The ROC curve was used to calculate the sensitivity 95.00%and specificity 82.70%of the CLIA method for the diagnosis of active tuberculosis.Conclusion CLIA method for detecting whole bloodγ-interferon has high sensitivity in the diagnosis of active tuberculosis,which is expected to become one of the main indicators for the auxiliary diagnosis of this disease.
