针灸对原发性骨质疏松症骨代谢影响的研究概述

Review on Effects of Acupuncture and Moxibustion on Bone Metabolism in Primary Osteoporosis

目的探讨针灸疗法治疗原发性骨质疏松症骨代谢的研究特点。方法翻阅在2008—2018年发表的有关针灸治疗对原发性骨质疏松症骨代谢影响的相关文献,并对针灸治疗的文献类型、疗效性、指导原则、骨代谢指标应用规律进行深度分析。结果发现在文献中以临床研究为主,机制研究和综述较少;针灸疗法对原发性骨质疏松症骨代谢具有改善作用,主要是针灸通过促进骨形成、抑制骨吸收的途径;指导原则以补肾健骨、健脾补肾调肝、活血化瘀为主;代谢指标中首推血清Ⅰ型前胶原氨基端前肽和骨吸收标志物中β-Ⅰ型胶原羧基端肽。结论针灸疗法对原发性骨质疏松症骨代谢的影响上,现在还处于初起阶段,临床研究逐渐增多,机制探索渐趋完善;在临床中疗效显著;指导原则基本确立;骨代谢指标中血清Ⅰ型前胶原氨基端前肽和骨吸收标志物中β-Ⅰ型胶原羧基端肽因其敏感性、动态监测性等今后将成为研究者的首选指标。

Objective To investigate the characteristics of acupuncture and moxibustion in the treatment of bone metabolism in primary osteoporosis.Methods We reviewed the literature on the effects of acupuncture and moxibustion treatment on bone metabolism of primary osteoporosis in 2008-2018,and carried out in-depth analysis of the literature types,therapeutic effects,guiding principles and application rules of bone metabolism indexes of acupuncture treatment.Results It was found that clinical research was the main study in the literature,and the mechanism was less studied and reviewed.Acupuncture and moxibustion treatment could promote bone formation and inhibit bone absorption.The guiding principle was to supplement the kidney,strengthen the spleen,regulate the liver and activate the blood.In the metabolic index,beta-I collagen carboxyl terminal peptide in serum type I anterior collagenamino terminal peptide and bone absorption marker was first pushed.Conclusion Acupuncture and moxibustion therapy on bone metabolism of primary osteoporosis is still in its early stages.The clinical research has been gradually increased,and the mechanism exploration has been gradually improved.The clinical effect is significant.The guiding principles are basically established.In the bone metabolism index,the serum type I anterior collagen amino terminal peptide and the beta-I collagen carboxyl terminal peptide in the bone absorption marker will be the first choice indicators for researchers due to their sensitivity and dynamic monitoring.
