临床麻醉中超声引导动脉穿刺置管的运用价值

Application value of ultrasound-guided arterial puncture catheter in clinical anesthesia

目的探讨临床麻醉中超声引导动脉穿刺置管的运用价值。方法选取本院2018年1月至2019年2月期间收治的术中行动脉穿刺置管患者60例为研究对象。采用随机数表法将所选患者分为对照组、观察组,给予对照组采用传统触摸法行左手桡动脉穿刺置管,给予观察组采用超声引导下桡动脉穿刺。对比观察两组患者一次穿刺成功率、穿刺总体失败率、穿透法制置管率及不良反应发生率、血流动力学指标等。结果经不同穿刺实施麻醉后结果可知,观察组患者一次穿刺成功率明显高于对照组患者(P<0.05);观察组穿刺总失败率、穿透法置管率、血管痉挛等不良反应发生率低于对照组(P<0.05);两组患者穿刺后平均动脉压、心率均高于穿刺前(P<0.05);但穿刺前后组间比较无明显差异(P>0.05)。结论超声引导下桡动脉穿刺置管在临床手术麻醉患者的应用,能明显降低患者的穿刺总统失败率和穿透法置管率,一次穿刺成功率较高,且不良反应率低,临床效果显著。

Objective To explore the value of ultrasound-guided arterial puncture catheterization in clinical anesthesia.Methods A total of 60 patients with intraoperative arterial puncture catheters admitted in our hospital from January 2018 to February 2019 were selected as the research subjects.Random number table method was used to divide the patients into a control group and an observation group.The control group was given a left-hand radial artery puncture via traditional touch method and the observation group was given a radial artery puncture under ultrasound guidance.The two groups of patients were compared to observe the success rate of one-time puncture,the overall failure rate of puncture,the rate of penetrating tube placement,the incidence of adverse reactions,and hemodynamic indicators.Results The results showed that the success rate of the primary puncture in the observation group was significantly higher than that of the control group(P<0.05).The rate of the total failure rate,the rate of penetration,the vasospasm and other adverse reactions in the observation group was lower than those of the control group(P<0.05).The average arterial pressure and heart rate of the two groups were higher than those before the puncture(P<0.05).However,there was no significant difference between the groups before and after the puncture(P>0.05).Conclusions The application of ultrasound-guided transarterial puncture in the patients with clinical operation can significantly reduce the failure rate of the puncture and the rate of penetration,the success rate of one-time puncture is high,and the rate of adverse reaction is low,and the clinical effect is remarkable.
