Objective To explore the effect of self-management mode on self-management status, quality of life and prognosis of patients with chronic heart failure. Methods 116 patients with chronic heart failure admitted to the friendship hospital of Urumqi from January 2019 to December 2019 were divided into two groups according to the random number table method, with 58 patients in each group. The two groups of patients received two self-management education training in the hospital. The observation group received the support of the mobile device-based self-management model after discharge, and compared the self-management status, quality of life, and prognosis of the two groups. Results There was no significant difference in the SCHFI and MLHFQ scores between the two groups 1 day before discharge(P>0.05).The SCHFI scores of the observation group at 3 and 6 months after discharge were higher than that of the control group, and the MLHFQ scores were lower than that of the control group. The difference was statistically significant(P<0.05).The acute attack rate and rehospitalization rate in the observation group were lower than that of the control group, and the differences were statistically significant(P<0.05).Conclusions The self-management model can significantly improve the self-management status of patients with chronic heart failure, improve the quality of life of patients, and benefit the prognosis.
