摘要 目的分析外周血调节性T细胞检测在再生障碍性贫血(再障)诊断中的价值。方法选择2018年4月至2020年3月于我院就诊的31再障患儿为研究对象,根据再障类型将其分成非重型再障组(21例)和重型再障组(10例);另选20例健康儿童为对照组。比较三组的外周血调节性T细胞水平、Foxp3 mRNA表达水平及TGF-β1水平,并分析再障患儿CD4+CD25+CD127low占CD4+T细胞百分比同Foxp3 mRNA、TGF-β1的相关性。结果非重型障组、重型再障组的CD4+CD25+占CD4+T细胞百分比、CD4+T细胞水平及CD4+CD25+CD127low占CD4+T细胞百分比均低于对照组,且重型再障组的低于非重型再障组,差异具有统计学意义(P<0.05)。对照组、非重型再障组的Foxp3 mRNA表达水平高于重型再障组,差异具有统计学意义(P<0.05)。非重型再障组、重型再障组的TGF-β1水平均低于对照组,且重型再障组的低于非重型再障组,差异具有统计学意义(P<0.05)。重型再障患儿的CD4+CD25+CD127low占CD4+T细胞百分比同Foxp3 mRNA呈正相关(P<0.05)。结论外周血调节性T细胞和Foxp3 mRNA水平下降同再障发生存在一定的关系,可为再障的病情变化提供理论依据。 Objective To analyze the value of peripheral blood regulatory T cells detection in the diagnosis of aplastic anemia.Methods Thirty-one children with aplastic anemia diagnosed in our hospital from April 2018 to March 2020 were selected as the research objects,and the children were divided into non-severe aplastic anemia group(21 cases)and severe aplastic anemia group(10 cases)according to the type of aplastic anemia;another 20 healthy children were selected as control group.The level of peripheral blood regulatory T cells,the expression level of Foxp3 mRNA and TGF-β1 level of the three groups were compared,and the correlation between the percentage of CD4+CD25+CD127low in CD4+T cells and Foxp3 mRNA and TGF-β1 was analyzed.Results The percentage of CD4+CD25+in CD4+T cells,the levels of CD4+T cells,and the percentage of CD4+CD25+CD127low in CD4+T cells in the non-severe aplastic anemia group and the severe aplastic anemia group were lower than those in the control group,and those in the severe aplastic anemia group were lower than the non-severe aplastic anemia group,and the differences were statistically significant(P<0.05).The expression level of Foxp3 mRNA in the control group and the non-severe aplastic anemia group was higher than that in the severe aplastic anemia group,and the differences were statistically significant(P<0.05).The level of TGF-β1 in the non-severe aplastic anemia group and the severe aplastic anemia group was lower than that in control group,that in the severe aplastic anemia group was lower than the the non-severe aplastic anemia group,and the differences were statistically significant(P<0.05).The percentage of CD4+CD25+CD127low in CD4+T cells of children with severe aplastic anemia was positively correlated with Foxp3 mRNA level(P<0.05).Conclusion The decrease of peripheral blood regulatory T cells and Foxp3 mRNA levels are related to the occurrence of aplastic anemia,which can provide a theoretical basis for the disease changes of aplastic anemia.
