2型糖尿病患者的肾糖阈及相关因素分析

Analysis of renal glucose threshold and related factors in patients with type 2 diabetes mellitus

目的探讨2型糖尿病患者肾糖阈水平及相关因素。方法选取本院内分泌科住院的107例2型糖尿病患者,以正常肾糖阈值(RTG,8.9~10 mmol/L)为切点,将患者分为高RTG组(RTG>10 mmol/L)、中等RTG组(8.9 mmol/L≤RTG≤10 mmol/L)和低RTG组(RTG<8.9 mmol/L),收集并分析各组临床资料及生化特征。结果2型糖尿病高、中、低RTG组患者比例为56%、29%、15%。高RTG组患者的RTG值、年龄、病程、体重指数(BMI)、空腹血糖(FPG)、HbA1C、总胆固醇(TC)、血肌酐、血糖平均值(MBG)及24 h尿糖与中等RTG组相比均有差异,高RTG组患者的RTG值、性别、BMI、FPG、HbA1C、TC及MBG与低RTG组相比有差异;中RTG组和低RTG组患者仅RTG值差异有统计学意义。相关分析显示,2型糖尿病患者性别、年龄、BMI、HbA1C、TC和低密度脂蛋白胆固醇(LDL-C)与RTG值呈正相关。回归分析显示,BMI、HbA1C和LDL-C是影响2型糖尿病患者RTG值的相关因素(均P<0.01)。结论2型糖尿病高RTG患者的比例大,BMI、HbA1C和LDL-C与2型糖尿病患者的RTG相关。

Objective To investigate the value of renal glucose threshold and related factors in patients with type 2 diabetes mellitus.Methods According to the cut-off point of normal renal glucose threshold(RTG 8.9-10 mmol/L),107 patients with type 2 diabetes mellitus hospitalized in the Endocrinology Department of our hospital were divided into three groups:high RTG group(RTG>10 mmol/L),medium RTG group(8.9 mmol/L≤RTG≤10 mmol/L),and low RTG group(RTG<8.9 mmol/L).The clinical data and biochemical characteristics of each group were collected and analyzed.Results The proportions of patients with high,medium,and low RTG of type 2 diabetes mellitus were 56%,29%,and 15%,respectively.There were significant differences in RTG value,age,course of disease,body mass index(BMI),fasting plasma glucose(FPG),HbA1C,total cholesterol(TC),serum creatinine,mean blood glucose(MBG),and 24-hour urine glucose between high and medium RTG groups.RTG,gender,BMI,FPG,HbA1C,TC,and MBG in patients with high RTG group were different from those in low RTG group.Only RTG revealed a difference between medium and low RTG groups.Correlation analysis showed that RTG was positively correlated with gender,age,BMI,HbA1C,TC,and low density lipoprotein-cholesterol(LDL-C).Regression analysis showed that BMI,HbA1C,and LDL-C were the related factors affecting the RTG of patients with type 2 diabetes.Conclusion There is a larger proportion of patients with high RTG in type 2 diabetes mellitus.Their BMI,HbA1C,and LDL-C are associated with RTG in the patients with type 2 diabetes mellitus.
