急性痛风患者疼痛与炎症因子相关性分析 被引量:4

摘要 目的:探讨痛风患者疼痛与炎症因子的相关性。方法:对2017年1月—2018年12月间在浙江省皮肤病医院痛风门诊就诊的72例急性痛风患者使用视觉模拟评分法(0~10分)评估其疼痛程度,并测定其血清IL-1β、IL-6、IL-8、TNF-α、hsCRP表达水平,使用SPSS17.0分析其相关性。结果:本组72例VAS积分平均为(5.070±2.016)分,轻度(VAS 1~3)、中度(VAS 4~6)和重度(VAS 7~10)疼痛患者分别占19.444%、51.389%和29.167%。TNF-α、IL-8、IL-6、hsCRP、IL-1β表达增多者分别占77.778%、70.833%、69.444%、25.000%和23.611%。经Spearman相关性分析,IL-8、TNF-α与VAS评分呈弱正相关,Spearman相关系数分别为:0.340和0.363,IL-1β、IL-6、hsCRP与VAS评分呈中等程度正相关,Spearman相关系数分别为:0.443,0.585和0.592。结论:痛风患者疼痛程度与血清IL-1β、IL-6、hsCRP表达水平之间有较好相关性,建议临床上将上述指标结合综合评判痛风病情严重程度。 Objective:To study the relationship between the pain and relevant inflammatory factors in gout patients.Methods:A total of 72 gout patients during the period January 2017 to December 2018 in gout clinic in Zhejiang Provincial Hospital of Dermatology,were evaluated on their pain degree by visual analogue scales(VAS).And their serum were selected and the levels of serum interleukin-1β,interleukin-6,interleukin-8,tumor necrosis factor-α,hsCRP were measured and the association were analyzed by Spearman correlation analysis.Results:The average VAS score of 72 patients is 5.070±2.016,with a mild(VAS 1~3),medium(VAS 4~6)and heavy(VAS 7~10)pain patients accounting for 19.444%,51.389%and 29.167%.And the level-increased factors of TNF-α,IL-8,IL-6,hsCRP,IL-1βaccounted for 77.778%,70.833%,69.444%,25.000%and 23.611%,respectively.There was a weak positive correlation between the VAS score and the level of serum interleukin-8,tumor necrosis factor-α,with Spearman correlation coefficients of 0.340 and 0.363,and a moderate positive correlation between VAS and IL-1β,IL-6,hsCRP,with Spearman correlation coefficients of 0.443,0.585 and 0.592,by Spearman correlation analysis in SPSS17.0.Conclusion:The pain degrees correlated well with the levels of IL-1β,IL-6 and hsCRP.It is recommended that the indicators be combined to evaluate the condition of gout patients.
