降钙素原(PCT)和C-反应蛋白(CRP)检测在糖尿病足感染临床诊断中的应用价值研究

目的探讨降钙素原(Procalcitonin,PCT)和C-反应蛋白(C-reactive protein,CRP)检测在糖尿病足感染临床诊断中的应用价值。方法回顾性分析了2016年8月至2019年8月期间锦州市中心医院收治的132例DF患者的临床资料,其中合并感染例数为46例,其余86例均为非感染DF患者。比较两组患者血糖相关指标(餐后2h血糖(Blood sugar 2hours after meal,2hPG))、糖化血红蛋白(Hemoglobin A1c,HbA1c)、血脂指标(甘油三酯(Triglycerides,TG))、血清PCT及CRP水平、二者检测DF合并感染的灵敏度及特异性。结果 (1)两组患者血糖指标(2hPG及HbA1c)及甘油三酯(TG)水平差异均无统计学意义(P均>0.05);(2)经Kruskal-Wallis检验,合并感染组患者血清中PCT及CRP水平均分别显著高于未合并感染组(P均<0.05);(3)血清中PCT及CRP对DF合并感染临床诊断的特异性及灵敏度均处于较高水平。结论 PCT和CRP检测在DF合并感染临床诊断中的应用价值较高,且二者检测灵敏度也较高,可作为临床诊疗DF合并感染的重要依据。

Objective To explore the application value of procalcitonin(PCT)and C-reactive protein(CRP)in the clinical diagnosis of diabetic foot infection.Methods The clinical data of 132 patients with DF admitted to our hospital from August 2016 to August 2019 were retrospectively analyzed.Among them,46 cases were combined with infection,and the remaining 86 cases were non-infected DF patients.Compare the blood glucose-related indicators(Blood sugar 2 hours after meal,2 hPG),Hemoglobin A1 c(HbA1 c),blood lipid indicators(Triglycerides,TG),serum PCT and CRP levels between the two groups of patients,The sensitivity and specificity of the two in detecting DF co-infection.Results(1)There was no statistically significant difference in blood glucose indexes(2 hPG and HbA1 c)and triglyceride(TG)levels between the two groups of patients(all P>0.05);(2)According to Kruskal-Wallis test,the levels of PCT and CRP in the serum of patients in the co-infection group were significantly higher than those in the non-co-infection group(P<0.05);(3)PCT and CRP in the serum were specific for the clinical diagnosis of DF co-infection Both the sensitivity and sensitivity are at a high level.Conclusion PCT and CRP detection have high application value in the clinical diagnosis of DF co-infection,and the detection sensitivity of both is also high,which can be used as an important basis for clinical diagnosis and treatment of DF co-infection.
