合并哮喘的阻塞性睡眠呼吸暂停低通气综合征患儿手术治疗效果观察

Therapeutic Effect Observation of Children with Asthma and Obstructive Sleep Apnea Syndrome

目的:探讨手术治疗对阻塞性睡眠呼吸暂停低通气综合征(OSAHS)儿童患者的临床价值及手术治疗OSAHS对儿童患者哮喘症状的改善效果。方法:选取山东省临沂市妇幼保健院儿科收治的OSAHS伴哮喘患儿78例作为研究对象,按照随机数字表法随机分为观察组和对照组,每组有39例。观察组进行手术治疗,对照组进行药物保守治疗。对观察组患儿进行治疗前后多道睡眠图(PSG)监测;对2组患儿哮喘症状改善情况评估比较。结果:手术治疗后,观察组OSAHS患儿的睡眠结构和呼吸事件得到有效改善,并且观察组患儿哮喘症状改善情况及有效率均显著高于对照组。观察组手术治疗前后睡眠结构和呼吸事件比较,差异有统计学意义(P <0.01),2组患儿哮喘症状改善情况及有效率比较,差异有统计学意义(P <0.05)。结论:本研究结果显示手术治疗对治疗儿童OSAHS具有较好的效果,且手术治疗儿童OSAHS对控制哮喘症状有积极意义。

Objective: To investigate the clinical value of surgical treatment of children with Obstructive sleep apnea hypopnea syndrome( OSAHS) and the significance of surgical treatment of OSAHS in improving asthma symptoms in children. Methods: Seventyeight children with Osahs and asthma who met the inclusion criteria were selected as the study subjects. The patients were randomly divided into the observation group and the control group with 39 cases in each group. The observation group was operated,the control group was treated conservatively. The children in the control group were monitored by PSG before and after treatment and the improvement of asthma symptoms was evaluated The Observation Group of children with asthma symptoms before and after treatment evaluation. Results: After the operation,the sleep structure and breathing events of the OSAHS children in the control group were improved effectively,and the improvement of asthma symptoms and effective rate in the control group were significantly higher than those in the observation group. There were significant differences in sleep structure and breathing events between the two groups before and after Operation( P < 0. 01). The improvement of asthma symptoms and the effective rate in the two groups were significant( P < 0. 05). Conclusion: The results of this study show that surgical treatment of children with Osahs has a good effect,and surgical treatment of children with Osahs has a positive role in controlling the symptoms of asthma.
