76例早产儿迟发型败血症发生前的睡眠时间及24 h心率变异性的特点分析

摘要 目的:探讨早产儿迟发型败血症发生前的睡眠时间,24 h的心率变异性特点分析及对其诊断价值。方法:回顾性收集2017年3月至2020年8月福建医科大学附属龙岩第一医院收治的早产儿迟发型败血症患儿76例作为研究对象,并选取出生孕周及体质量相似的非迟发型败血症患儿76例作为对照组。统计2组的睡眠时间,并分析SDNN、SDNNIDX及SDANN单一诊断及联合诊断时的效能。结果:观察组的睡眠时间长于对照组,差异有统计学意义(P <0.05)。SDNN、SDNNIDX及SDANN单独及联合评估的AUC面积分别为0.82、0.77、0.71、0.93。结论:败血症早产儿易嗜睡,SDNN、SDNNIDX及SDANN对早产儿迟发性败血症均有一定诊断价值,但三者联合效能最佳。 Objective: To investigate the characteristics of sleep time and heart rate variability 24 h before the onset of delayed sepsis in premature infants and its diagnostic value. Methods: A total of 76 premature infants with delayed septicemia were retrospectively collected from March 2017 to 2020 in Longyan First Hospital Affiliated to Fujian Medical University and 76 non-delayed septicemia infants with similar gestational age and weight were selected as the control group. The sleep time of the two groups was counted,and the efficacy of SDNN,SDNNIDX and SDANN in single diagnosis and combined diagnosis was analyzed. Results: The sleep time of the observation group was longer than that of the control group,and the difference was statistically significant( P <0. 05). The AUC areas assessed separately and jointly by SDNN,SDNNIDX and SDANN were 0. 82,0. 77,0. 71 and 0. 93,respectively. Conclusion: Premature infants with septicemia are prone to drowsiness. SDNN,SDNNIDX and SDANN have certain diagnostic value for delayed septicemia in premature infants,but the combined efficacy of the three is the best.
