摘要
目的:观察磺胺嘧啶银联合红外线对重症加强护理病房(ICU)重度压疮(Pu)患者创伤面积与疼痛程度的影响。方法:选择2016年3月—2019年3月收治的50例ICU重度Pu患者,依据随机数表法分成观察组和对照组,每组25例。对照组采用红外线和碘伏进行治疗,观察组给予红外线联合磺胺嘧啶银治疗。观察治疗前及治疗14 d后两组患者疗效、疼痛程度[视觉模拟评分法(VAS)]、压疮风险[压疮评分量表(Braden)]和血生化指标[血红蛋白(Hb)、血清白蛋白(Alb)、C反应蛋白(CRP)]变化,比较治疗7 d和14 d后两组患者创伤面积及愈合时间的差异。结果:治疗14 d后,观察组患者治疗总有效率明显高于对照组(P<0.05);两组患者VAS评分及CRP水平均较治疗前显著降低,且观察组明显低于同一时间对照组;Braden评分、Hb和Alb水平均较治疗前显著升高,且观察组明显高于同一时间对照组(P<0.05)。治疗7 d,14 d后,观察组患者创伤面积明显低于同一时间对照组,且观察组患者愈合时间明显短于对照组(P<0.05)。结论:磺胺嘧啶银联合红外线应用于ICU重度Pu患者疗效良好,创伤面积缩小趋势明显,愈合时间短,疼痛程度随之减轻,机体营养状态得到改善,且可有效抑制炎症反应,有利于患者预后恢复。
Objective:To observe the effects of silver sulfadiazine combined with infrared on wound area and pain level of patients with severe pressure ulcer(Pu)in intensive care unit(ICU).Methods:Fifty patients with severe Pu who were admitted to ICU of hospital were selected as the study subjects.They were divided into experimental group and control group according to the random number table method,with 25 cases in each group.Control group was treated with infrared and iodophor while experimental group was given infrared combined with silver sulfadiazine.The efficacy,pain level[visual analogue scale(VAS)],risk of pressure ulcer[pressure ulcer score scale(Braden)]and blood biochemical indicators[hemoglobin(Hb),serum albumin(Alb),C-reactive protein(CRP)]were observed in the two groups before treatment and after 14 d of treatment.The wound area and healing time were compared between the two groups after 7 d and 14 d of treatment.Results:After 14 d of treatment,the total effective rate of treatment in experimental group was significantly higher than that in control group(P<0.05).The VAS score and CRP level in the two groups were significantly decreased compared with those before treatment,and the two indexes in experimental group were significantly lower than those in control group at the same time point.The Braden score and levels of Hb and Alb were significantly increased compared with those before treatment,and the three indexes in experimental group were significantly higher than those in control group at the same time point(all P<0.05).After 7 d and 14 d of treatment,the wound area in experimental group was significantly lower than that in control group at the same time point,and the healing time in experimental group was significantly shorter than that in control group(all P<0.05).Conclusion:Silver sulfadiazine combined with infrared can achieve good efficacy in patients with severe Pu in ICU.And it can significantly reduce wound area of patients,and has shorter healing time,and relieve the pain level,improve the body′s nutritional status,and effectively suppress the inflammatory response.What′s more,it is conducive to the prognosis and recovery of patients.
