广州市某社区2型糖尿病患者抑郁发病率及其相关因素分析

Prevalence of Depression in Patients with Type 2 Diabetes Mellitus in Community of Guangzhou and its Risk Factors

目的:探索广州市某社区2型糖尿病患者中抑郁发作的患病率以及相关的影响因素。方法:采用连续入组2型糖尿病患者308例,应用一般情况调查表、社会支持量表(SSRS)、抑郁自评量表(SDS)、焦虑自评量表(SAS)调查2型糖尿病患者中抑郁发作的患病率,并通过Logistic回归方法分析伴有抑郁障碍的患者相关影响因素。结果:2型糖尿病患者的抑郁评分为(51.1±6.5),患病率为41.9%。2型糖尿病合并抑郁患者的焦虑程度高于无抑郁患者(P<0.01),而社会支持水平则低于不伴抑郁发作的患者(P<0.01)。Logistic回归分析显示,体重指数(OR=4.527,95%CI:2.2~5.5)、空腹血糖水平(OR=2.613,95%CI:1.0~7.1)、焦虑症状(OR=4.368,95%CI:2.8~7.5)是伴有抑郁发作患者的独立危险因素。结论:2型糖尿病患者中抑郁发作的患病率较高,社会支持水平低,肥胖、血糖控制欠佳及伴有焦虑症状的患者更易出现抑郁障碍,糖尿病的社区常规管理要对此加以关注。

Objective:To examine the prevalence of depression and its risk factors in patients with type 2 diabetes mellitus in Guangzhou.Method:A total of 308 patients with type 2 diabetes were consecutively enrolled in this study.The general information questionnaire,self-rating depression scale(SDS),self-rating anxiety scale(SAS)and social support rating scale(SSRS)were used to calculate the prevalence of depression,and Logistic regression analysis was performed to identify the influencing factors for depression in patients with type 2 diabetes.Results:SDS total scores were 51.1±6.5,and the prevalence of depression was 41.9%.The anxiety degree in type 2 diabetes patients with depression were significantly higher than those of patients without depression,but the social support level was significantly lower in type 2 diabetes patients with than without depression.Logistic regression analysis showed that body mass index(OR=4.527,95%CI:2.2-5.5),fasting blood glucose level(OR=2.613,95%CI:1.0-7.1)and anxiety degree(OR=4.368,95%CI:2.8-7.5)were independent risk factors of depression in patients with type 2 diabetes.Conclusions:Depression can be easily observed in patients with type 2 diabetes,and the social support level was lower in type 2 diabetic patients with than without depression.Type 2 diabetic patients with obesity,bad blood glucose control and anxiety were more susceptible to depression.We should pay more attention this phenomenon in the community management of diabetes.
