中性粒细胞减少肿瘤患者抗菌药物临床用药点评

Comment on clinical use of antibiotics in patients with neutropenia

目的:针对中性粒细胞减少肿瘤患者用药实施专项处方点评,评价临床用药合理性。方法:随机抽取2015年4月—2019年3月诊断含“恶性肿瘤”、在院治疗期间出现Ⅳ度中性粒细胞减少并使用抗菌药物400例患者的临床资料,进行专项点评。结果:实际共抽取病例396例,其中226例出现发热,均使用抗菌药物进行初始经验性治疗。总体存在用药品种不合理72例(31.86%),用法用量不合理28例(12.39%),用药途径不合理8例(3.54%)。结论:中性粒细胞减少肿瘤患者中存在抗菌药物用药不合理情况,应加以重视。

Objective:To evaluate the rationality of clinical medication for patients with neutropenia.Methods:From 2015 April 1 to 2019March 31,400 cases of inpatient with‘malignant tumor’and IV degree Neutropenia were randomly selected and the prescriptions were reviewed.Results:A total of 396 cases were selected,among them,226 had fever,all of them were treated with antibiotics.There are inappropriate antibiotic selection(72 cases,31.86%),inappropriate dosage(28 cases,12.39%)and inappropriateroute of administration(8 cases,3.54%).Conclusion:in the patients with neutropenia tumor,the use of antibiotics is not appropriate and should be paid attention to.
