摘要
目的研究>40岁人群腰臀比、尿素氮、胰岛素抵抗与China-PAP模型10年心血管病发病风险评估结果的关系。方法按照随机抽样的原则,抽取河北省2所大学及7家政府机关事业单位未患心血管疾病、恶性肿瘤、慢性肝肾病、急性感染性疾病>40岁人群为研究对象,采集腰围、臀围、身高、体重、血压、血生化、血常规、胰岛素等相关数据及完善问卷。利用China-PAP模型进行心血管病10年发病风险评估,按照评估结果分高、中、低危3组。3组之间差异的比较计量资料采用方差分析,计数资料采用χ^2检验;应用多重线性回归分析各项指标与心血管病发病风险之间的关系。风险评估模型中纳入的指标未做统计学比较。结果除血白蛋白、球蛋白及血小板(PLT)外,体重指数、臀围、腰臀比、血尿酸、低密度脂蛋白、三酰甘油(TG)、肌酐、尿素氮(BUN)、红细胞、血红蛋白、白细胞、胰岛素抵抗对数从低危组到高危组均逐步升高,3组之间差异均有统计学意义(P<0.05)。通过多重线性回归分析发现,对心血管病发病风险有影响的变量为log(HOMA-IR)(X1)、TG(X2)、BUN(X3)、臀围(X4)、腰臀比(X5),回归方程为Y=2.819X1+0.764X2+0.877X3+0.255X4+27.551-35.233,标准化回归系数腰臀比0.320、臀围0.304、BUN 0.167、TG 0.144、log(HOMA-IR)0.140。结论>40岁人群臀围、腰臀比、甘油三酯、尿素氮、胰岛素抵抗的升高可能会增加心血管病发病风险。
Objective To investigate the correlation between waist hip ratio,urea nitrogen,insulin resistance and the onset risk of cardiovascular disease in people over 40 years of age according to the assessment results of China PAP model in 10 years.Methods According to the principle of random sampling,the people with over 40 years of age who were not suffered from cardiovascular disease,malignant tumor,chronic liver and kidney disease,acute infectious disease in two universities and seven government agencies and institutions in Hebei province were enrolled in the study.The relevant data including waist circumference,hip circumference,height,weight,blood pressure,blood biochemistry,blood routine,insulin use were collected to improve the questionnaire.The China-PAP model was used to perform a 10-year risk assessment of cardiovascular disease,according to the assessment results,these subjects were divided into high,medium and low risk groups.Multiple linear regression was used to analyze the relationship between various indexes and the onset risk of cardiovascular disease.Results Except for albumin,globulin and platelets,the body mass index,hip circumference,waist-to-hip ratio,blood uric acid,low density lipoprotein,triglyceride(TG),creatinine,urea nitrogen(BUN),red blood cells,hemoglobin,white blood cell and the logarithm of insulin resistance were increased gradually from the low-risk group to the high-risk group,there were significant differences among the three groups(P<0.05).Through multiple linear regression analysis,the variables that had effects on the onset risk of cardiovascular disease were log(HOMA-IR)(X1),TG(X2),BUN(X3),hip circumference(X4),waist-to-hip ratio(X5).The regression equation was Y=2.819X1+0.764X2+0.877X3+0.255X4+27.551 X5-35.233,and the standardized regression coefficients waist-to-hip ratio was 0.320,hip circumference 0.304,BUN 0.167,TG 0.144,log(HOMA-IR)0.140.Conclusion The increase in hip circumference,waist-to-hip ratio,triglyceride,urea nitrogen,and insulin resistance in people aged over 40 years may increase the onset risk of cardiovascular disease.
