慢性心力衰竭患者血浆外泌体miR-214-3p和miR-184的表达及意义 被引量:5

摘要 目的探讨慢性心力衰竭(CHF)患者血浆外泌体miR-214-3p、miR-184表达及意义。方法心力衰竭患者106例(心衰组)及健康者127例(对照组),利用实时定量PCR(qRT-PCR)对所有研究对象血浆外泌体miR-214-3p、miR-184表达水平进行检测,并分析其与临床特征[左心室舒张末期内径(LVEDD)、左心室短轴缩短率(FS)、左心室射血分数(LVEF)、血浆脑钠肽(BNR)]之间关系。结果CHF患者血浆外泌体miR-184表达水平显著低于对照组,miR-214-3p表达水平显著高于对照组,差异均有统计学意义(P<0.05)。不同心功能分级患者BNP、LVEDD、FS、LVEF水平,与对照组相比较差异有统计学意义(P<0.05)。随NYHA心功能分级(Ⅱ~Ⅳ)增加,CHF患者miR-184表达水平及FS、LVEF水平逐级降低,miR-214-3p表达水平及BNP、LVEDD水平逐级增高(P<0.05)。Pearson相关分析显示,CHF患者miR-214-3p表达水平与BNP、LVEDD呈正相关(r值分别为0.549、0.427,P<0.05),与LVEF呈负相关(r=-0.413,P<0.05);CHF患者miR-184表达水平与BNP、LVEDD呈负相关(r值分别为-0.525、-0.483,P<0.05),与LVEF呈正相关(r=0.397,P<0.05)。结论CHF患者血浆外泌体miR-214-3p表达水平升高,miR-184表达水平降低,与患者心功能有关,可能参与CHF疾病发生发展。 Objective To investigate the expressions and significance of plasma exosome-miR-214-3p and miR-184 in patients with chronic heart failure(CHF).Methods A total of 106 patients with CHF who were treated in our hospital from January 2016 to December 2018 were enrolled as CHF group,at the same time,the other 127 healthy subjects were enrolled as control group.The expression levels of miR-214-3p and miR-184 in plasma exosomes were detected by real-time quantitative PCR(qRT-PCR),and the correlation between the expressions and clinical indexes(LVEDD,FS,LVEF,BNR)of CHF was analyzed.Results The expression levels of miR-184 in plasma exosome in CHF group were significantly lower than those in control group,but the expression levels of miR-214-3p were significantly higher than those in control group(P<0.05).There were significant differences in the expression levels of BNP,LVEDD,FS and LVEF in the patients with different cardiac function grades between CHF group and control group(P<0.05).With the increase of NYHA cardiac function classification(Ⅱ~Ⅳ),the expression levels of miR-184,and the levels of FS and LVEF were decreased gradually,however,the expression levels of miR-214-3p,and the levels of BNP and LVEDD were increased gradually in CHF group(P<0.05).Pearson correlation analysis showed that the expression levels of miR-214-3p in CHF group were positively correlated with those of BNP and LVEDD(P<0.05),however,which were negatively correlated with those of LVEF(P<0.05).Moreover the expression levels of miR-184 in CHF group were negatively correlated with those of BNP and LVEDD(P<0.05),however,which were positively correlated with those of LVEF(P<0.05).Conclusion The expression levels of miR-214-3p in plasma exosome of patients with CHF are increased,and the expression levels of miR-184 are decreased,which is related to patients’cardiac function and may be involved in the occurrence and development of CHF.
