摘要
目的探讨Ⅰ期肺腺癌^(18)F-FDG PET/CT表现与Ki-67表达的相关性。方法回顾性分析56例Ⅰ期肺腺癌患者的^(18)F-FDG PET/CT及病理学资料,将其分为纯磨玻璃结节(pGGN)组(15例)、混杂磨玻璃结节(mGGN)组(11例)及实性结节组(30例),观察其PET/CT征象[包括最大标准摄取值(SUV_(max))、结节体积、磨玻璃影(GGO)百分比、毛刺征、分叶征、胸膜凹陷征、血管集束征及支气管截断征]与Ki-67表达之间的相关性。结果实性结节组SUV_(max)明显高于mGGN组和pGGN组(P均<0.05),mGGN组SUV_(max)明显高于pGGN组(P<0.05);实性结节组Ki-67明显高于mGGN组和pGGN组(P均<0.05)。Ⅰ期肺腺癌Ki-67阳性表达与SUV_(max)呈正相关(P<0.05),与GGO百分比及毛刺征均呈负相关(P均<0.05),而与肿瘤体积、分叶征、血管集束征、胸膜凹陷征及支气管截断征均无明显相关(P均>0.05)。SUV_(max)与肿瘤体积呈正相关(P<0.05),与GGO百分比呈负相关(P<0.05),与毛刺征、血管集束征、胸膜凹陷征及支气管截断征均存在相关性(P均<0.05),与分叶征无明显相关(P>0.05)。结论Ⅰ期肺腺癌Ki-67阳性表达与^(18)F-FDG PET/CT所示SUV_(max)及肿瘤实性成分比例正相关;SUV_(max)与毛刺征、胸膜凹陷征、血管集束征及支气管截断征均存在相关性。
Objective To investigate the correlation of ^(18)F-FDG PET/CT manifestations and the expression of Ki-67 in stageⅠlung adenocarcinoma.Methods ^(18)F-FDG PET/CT and pathological data of 56 patients with stageⅠlung adenocarcinoma were retrospectively analyzed.The nodules were divided into pure ground-glass nodules(pGGN)group(n=15),mixed ground-glass nodules(mGGN)group(n=11)and solid nodules group(n=30).The correlation of PET/CT manifestations,including maximum standardized uptake value(SUV_(max)),nodule volume,percentage of ground-glass opacity(GGO),spiculation sign,lobulation sign,pleural indentation sign,vessel convergence sign and bronchial truncation sign with Ki-67 were analyzed.Results SUV_(max) was significantly higher in solid nodules group than that in mGGN group and pGGN group(both P<0.05),and in mGGN group was higher than in pGGN group(P<0.05).The expression of Ki-67 was significantly higher in solid nodules group than tin mGGN group and pGGN group(both P<0.05).Positive expression of Ki-67 positively correlated with SUV_(max) and negatively correlated with GGO percentage(both P<0.05),but not correlated with nodule volume,lobulation sign,vessel convergence sign,pleural indentation sign nor bronchial truncation sign(all P>0.05).SUV_(max) positively correlated with nodule volume and negatively correlated with GGO percentage(all P<0.05),while not correlated with spiculation sign,vessel convergence sign,pleural indentation sign nor bronchial truncation sign(all P<0.05).Conclusion Ki-67 positive expression of stageⅠlung adenocarcinoma were positively correlated with ^(18)F-FDG PET/CT SUV_(max) and the ratio of solid composition within the tumor.SUV_(max) of stageⅠlung adenocarcinoma correlated with spiculation sign,pleural indentation sign,vessel convergence sign and bronchial truncation sign.
