对比胃与小肠间质瘤多层螺旋CT征象及病理学特点

Comparison on multi-slice spiral CT manifestations and pathological features of gastric and small intestinal stromal tumors

目的对比观察胃间质瘤与小肠间质瘤多层螺旋CT(MSCT)征象及病理学特点。方法回顾性分析经病理学确诊的165例胃间质瘤(GST组)与37例小肠间质瘤(SIST组),对比观察其CT征象及病理学特点。结果GST组与SIST组肿瘤生长方式及形态差异均有统计学意义(P均<0.05)。GST组中95.76%(158/165)肿瘤增强CT三期强化方式为渐进性强化,SIST组89.19%(33/37)强化方式为速升-缓降型强化,2组肿瘤平扫及三期增强CT值差异均有统计学意义(P均<0.05)。SIST组病理学危险程度分级明显高于GST组(P<0.05),其CD34阳性率明显低于GST组(P<0.05),而2组肿瘤大小、内部情况、坏死、钙化及免疫组化指标CD117、DOG1、Ki-67差异均无统计学意义(P均>0.05)。结论GST与SIST虽同属胃肠道间质肿瘤,但其CT征象及病理学特点均存在一定差异。

Objective To compare multi-slice spiral CT(MSCT)manifestations and pathological features of gastric stromal tumor(GST)and small intestinal stromal tumor(SIST).Methods Data of 165 patients with GST(GST group)and 37 with small intestinal stromal tumor(SIST group)confirmed by pathological examination were retrospectively analyzed.CT manifestations and pathological features were observed and compared between groupd.Results There were significant differences of tumor growth pattern and morphology between GST group and SIST group(both P<0.05).During 3-phase enhanced CT,progressive enhancement was observed in 95.76%(158/165)cases of GST group,while“fast-rising-slow-descending”in 89.19%(33/37)of SIST group.There were statistically significant differences of CT value in plain scan and 3-phase enhancement between groups(all P<0.05).The pathological risk grades of SIST group were significantly higher than those of GST group(P<0.05).The positive rate of CD34 in SIST group was significantly lower than that in GST group(P<0.05).No significant difference of tumor size,internal condition,necrosis,calcification nor immunohistochemical indexes(CD117,DOG1 and Ki-67)was found between groups(all P>0.05).Conclusion CT manifestations and pathological features of GST and SIST were somehow different,though both being gastrointestinal stromal tumors.
