补中益气汤联合脐灸治疗气虚便秘临床疗效评价

Evaluation of clinical efficacy of Buzhong Yiqi Decoction combined with umbilical moxibustion on Qi deficiency constipation

目的观察补中益气汤联合脐灸治疗气虚便秘的临床效果。方法选取2019年1—8月石家庄市中医院老年病科收治的100例气虚便秘患者作为研究对象,随机分为对照组30例、补中益气组35例以及补中益气联合脐灸组35例,对照组患者应用黄芪颗粒治疗,补中益气组患者给予补中益气汤治疗,补中益气联合脐灸组患者给予补中益气汤联合脐灸治疗,3组均治疗4周。对比3组患者治疗前后排便指标(排便间隔时间、粪质、排便难度及排便速度评分)和血清胃动素(MTL)、P物质水平,统计3组临床疗效及随访6个月复发率。结果3组患者治疗后排便间隔时间、粪质、排便难度以及排便速度评分均较治疗前明显降低(P均<0.05),补中益气联合脐灸组明显低于补中益气组(P均<0.05),补中益气组明显低于对照组(P均<0.05);治疗后3组MTL、P物质水平均明显升高(P均<0.05),补中益气联合脐灸组明显高于补中益气组(P均<0.05),补中益气组明显高于对照组(P均<0.05);补中益气联合脐灸组治疗总有效率明显高于补中益气组(P<0.05),补中益气组明显高于对照组(P<0.05);补中益气联合脐灸组、补中益气组、对照组复发率依次明显下降(P<0.05)。结论补中益气汤联合脐灸治疗气虚便秘可明显改善患者排便质量,降低复发率,机制可能与调节机体MTL、P物质水平有关。

Objective It is to observe the clinical effect of Buzhong Yiqi Decoction combined with umbilical moxibustion on Qi deficiency constipation.Methods A total of 100 patients with Qi deficiency constipation admitted to the Department of Geriatrics of Shijiazhuang Hospital of Traditional Chinese Medicine from January to August 2019 were selected as the research objects.They were randomly divided into control group(n=30),Buzhong Yiqi group(n=35)and combination group(n=35),the patients in the control group were treated with Huangqi granules,the patients in the Buzhong Yiqi group were treated with Buzhong Yiqi Decoction,and the patients in the combination group were treated with Buzhong Yiqi Decoction and umbilical moxibustion.All the 3 groups were treated for 4 weeks.The defecation indicators(defecation interval,stool quality,defecation difficulty and defecation speed score)and the levels of serum motilin(MTL)and substance P were before and after treatment were compared in the three groups,the clinical efficacy and recurrence rate after 6 month’s follow-up of the three groups were counted.Results The scores of defecation interval,stool quality,defecation difficulty,and defecation speed after treatment in the three groups were significantly lower than those before treatment(P<0.05).The scores in the combination group were lower than those in the Buzhong Yiqi group(P<0.05),and the Buzhong Yiqi group was lower than the control group(P<0.05).After treatment,the levels of MTL and substance P in the three groups were significantly increased(P<0.05),the levels in the combination group were significantly higher than those in the Buzhong Yiqi group(P<0.05),and the Buzhong Yiqi group was significantly higher than the control group(P<0.05);the total effective rate of the treatment in the combination group was significantly higher than that in Buzhong Yiqi group(P<0.05),and the Buzhong Yiqi group was significantly higher than the control group(P<0.05);the recurrence rate of combination group was lower than that in the Buzhong Yiqi group,and Buzhong Yiqi group was lower than control group(P<0.05).Conclusion Buzhong Yiqi Decoction combined with umbilical moxibustion can significantly improve the quality of defecation and reduce the recurrence rate in the treatment of Qi-deficiency constipation.The mechanism may be related to the regulation of the levels of body’s MTL and substance P.
